Someone in my last Q&A asked how to get his wife to stop asking for a third baby.

I don’t have a great answer for that one.

In my experience, in that particular negotiation, the wife tends to win. It probably doesn’t help you made great kids so she’s thinking why not continue the perfection!! Serves you right for having great genes!

Someone else just said: “Not a question. Just please don’t retire for a long time.”
That one I can actually answer.

I have no plans to go anywhere. I am still loving this work. Still feel fit, healthy, and capable. And if my contribution is valued, I am happy to keep showing up.

You can count on at least another 10 years from me.

What if a caesarean could feel like a natural birth moment?

Years ago, I cared for a woman coming to me for her second pregnancy. Her first birth had been an emergency caesarean, and what stayed with her wasn’t the surgery. It was the disconnect, the moment her baby was born and she couldn’t reach it.

Traditionally, a screen separates you from the operative field. Baby is delivered, shown to you briefly, then taken away to be dried and wrapped. Several minutes can pass before you get that first cuddle. After nine months of nurturing that baby, that gap matters.

When I discovered maternally assisted caesarean sections, I knew I had to offer this.

Without a screen, I deliver the baby in a fully sterile way. Once baby is halfway out, I guide your gloved hands down so you can bring your baby onto your chest yourself, immediately. Skin on skin making that first moment, yours.

I’ve done this in elective and emergency situations, with twins, with breeches, with low grade praevias, and with partners scrubbing in to help. If you’re planning a caesarean, please ask your obstetrician about this option. It is one of the most meaningful things I’ve introduced in over 30 years of practice.

One of the most common questions I get: “How can I bring on labor naturally to avoid induction?”

I wish I had a magic answer.
The truth is, despite what you may have heard about dates, raspberry leaf tea, chili meals or acupuncture, there is no naturopathic approach that has been scientifically proven to bring on labor. No harm in trying them, but I would not guarantee results.

And if you do need to be induced, it is because there is a clinical reason. You are either overdue, or there is a concern with blood pressure, blood sugar, the baby’s growth, or something else. Induction is always a decision made with your safety and your baby’s safety in mind.

Talk to your care provider, ask the questions, and trust the process.

Gestational diabetes + night shifts in your third trimester. A lot going on, right?

The good news: gestational diabetes, whether managed through diet alone (80% of cases) or with low dose insulin (20%), should not significantly impact your baby’s health.

You don’t have to push through everything. Advocate for yourself and your baby.

Diabetes and work: Often a sensible conversation with your employer re work-life balance and managing pregnancy will suffice but do speak to your care provider for a medical certificate where appropriate.

Thinking about Botox or fillers during pregnancy? 💉🤰

Here’s my take:
Botox and fillers act locally. Botox paralyzes specific muscles, like those in the forehead to lose frown lines, and fillers are placed in a targeted spot.

These treatments do not travel systemically through your body or reach the baby. Scientifically, there is no reason they couldn’t be used during pregnancy.

However, there isn’t a huge amount of research confirming they are “absolutely safe”. Because of this, it is very unlikely you will find a care provider willing to perform these injections while you are pregnant.

While likely safe in principle, in practice, you should just wait until the end of your pregnancy.

Last week I had the privilege of joining colleagues Chelsea, Renee and Helen for the GP education evening, where I spoke on the topic of contraceptive pill use in gynecology.

Evenings like this are a reminder of how important ongoing education is across every corner of women’s health. A great night, and a pleasure to have been involved.

While today is a moment many people pause to recognise women, I’m reminded every day, both in my work and at home, of the strength, resilience and impact women have.

In my work I have the privilege of caring for women at some of the most significant moments in their lives. It is a responsibility I never take lightly, and something that continues to shape the way I see the world.

But today my thoughts are also very much at home.

As a husband, I am constantly grateful for my wife Mandy. Her strength, perspective and unwavering support have shaped my life in ways that are difficult to put into words.

As a father, watching our daughters grow into their own lives and futures is one of the greatest privileges I have.

And I’m also grateful for my mother, whose guidance and influence continue to shape who I am.

To all the women in Melbourne and beyond, thank you for everything you bring to your families, workplaces and communities. It may not always be said out loud, but you are deeply valued. Our lives, families and communities would not be what they are without you.

Happy International Women’s Day.

Whilst meeting Anan late in her pregnancy it was a joy to be involved in a beautiful birth, one that completed her family and was a high point in her birthing cycle. I am just so pleased. Congrats on Ainaan’s safe arrival and be well.
