Port Hedland Family Practice is PERMANENTLY CLOSED!

If you require your medical notes to be transferred to your NEW MEDICAL CENTRE, please contact them and fill out a request to transfer your notes.
There will be a fee of $25.00 for all future transfers.

The Port Hedland Family Practice has closed PERMANENTLY!

Any patient requiring their notes to be transferred to another clinic will need to complete a request form from their new clinic and submit prior to 26th October.
After this date the server will be archived.

The Port Hedland Family Practice will be open from Tuesday 31st each week day between 9am and 11am for patients to bring in their transfer request forms.
We will transfer your file to your new clinic of choice for FREE.

Any enquiries please call the clinic between 9 and 11am.

Dear Members of the Port Hedland Community,

It is with great regret we wish to advise that the Port Hedland Family Practice is permanently closed as of 27.5.2022.

For patients requiring to have their Medical history transferred to an alternative Medical Centre please contact your new clinic and complete a transfer request form.
On the condition that our adminstration staff are covid free the clinic will be open from Tuesday 31.5.2.22 between the hours of 9am to 11am to deal with any enquiries. There will be no doctor on site.
