The fourth and final part of the dementia care series. How does OT intertwine with dementia care?
Interventions for those with dementia include:

• Health promotion - focusing on maintained strengths of clients and promoting wellness of care providers.

• Remediation. Although the remediation of cognitive skills is not expected, routine exercises can be used to improve the performance of ADLs and functional mobility.

• Maintenance- Provide supports for the habits and routines that are working well for the person with dementia, and that can be maintained to prolong independence.

• Modification - It ensures a safe and supportive environment through adaptation and compensation, including verbal cueing, personal assistance, and/ or social supports.

By setting goals with the client, OTs can narrow their focus and provide highly targeted interventions. Client satisfaction with services is improved because they are seeing results in the areas that matter most to them.

Questions to ask when writing SMART goals:
Specific: who, what, when, where and why? What will the patient or caregiver do? Avoid vague descriptions.

Measurable: How will I measure progress? How will I know it is achieved? Use validated and objective measures.

Actionable/Attainable: Is the goal reasonable and achievable? Can this really happen?

Relevant: Why is achieving this goal important? Is the goal meaningful? Establish goals in partnership with your patient/caregiver.

Time Bound: When will the goal be achieved? What is the timeframe for achieving the goal? Set a deadline and avoid “by discharge”.
