Care Plans have a new name ➡ GP Chronic Care Management Plans!

Long name but means some great changes to help you both set up and manage your chronic conditions.

Your GPCCMP now makes it:
💜 Easier: No longer requires input from two outside providers to start,
💜 Flexible: Can be done in person or via video consultation,
💜 Ongoing: Plans don't expire, but must be reviewed every 18 months,
💜 More Support: Reviews can happen every 3 months if needed.

Dementia Action Week 15th - 21st September

The term dementia covers many conditions such as Alzheimers and vascular dementia to name a few but is important to understand that its effects differ from person to person.
