ANOTHER GCKG PAPER PUBLISHED

Thanks to the help of Larissa and Zach for their contribution towards this paper.

From data collected during the original version of our Sports Knee Program we learnt a lot. Things changed a lot since then and we now believe we offer the best environment for semi-professional, amateur or recreational level athletes to get the training they need to return to sport.

We knew and the research supports that we can make big changes to a persons function with a structured progressive program in a group environment. However, short time frames close to return to sport are not enough. You need consistent participation in a well designed program to truly hit your goals.

PREHABILITATION’ before your knee replacement surgery – Useful or a waste of time?
 
We actually don’t have strong evidence to support prehab before knee repleacment. the use of a structured prehabilitation

However, it is likely worth discussing the following with your physio: 

1. advice and education around pain/swelling management, diet/nutrition and physical activity
 
2. strength-based exercise

KIDS ACL INJURIES INCREASING WORLD WIDE

More and more reports continue to come out highlighting the increase in ACl injuries in kids.

This study helps explain?

The incidence of pediatric ACLRs in Norway increased from 18 to 26 per 100,000 population, corresponding to an increase of 40% for boys and 55% for girls from 2005 to 2021.

The increased incidence of ACLR in our study was consistent with reports from the United States, Australia, and Finland.

The cause of the increase in ACLR, and the possible increase in ACL injuries, is probably multifactorial.

With 96% of the injuries being sports-related, the increase may be attributed to earlier sports specialization, with intense training programs and a higher level of competition at a younger age, potentially limiting free play and exposure to adversity.

The increase in ACLR may also be a result of diagnostic improvements (magnetic resonance imaging) and public awareness of both the injury and the reported association with poorer outcomes in patients who did not undergo operation.

It could also be explained by the advancement of techniques and equipment required for skeletally immature patients and the subsequently increased number of orthopaedic surgeons with experience in performing these surgical procedures.

BRA TYPE INCREASES ACL INJURY RISK???

Got an interesting one for this week!

From the ABC….
An international team of experts in biomechanics and sports medicine decided to take the task on and studied 35 female recreational athletes.

They found when women wore highly supportive bras while completing jumping and landing tasks, many of the common risk factors associated with ACL injuries decreased.

Knee flexion angles were smaller, which means the knee didn’t have to bend as much on landing, and there was also a reduction in dynamic knee valgus, which is when the knee moves inwards from the foot.

Lead researcher Douglas Powell, director of the Breast Biomechanics Research Center at the University of Memphis, found the results surprising.

We’re talking about a meaningful level of [stress] on the ACL.”

My take…
It is a pretty wild step to take from a small study looking at biomechanics of risk factors that may be associated with ACL injury risk to extrapolate the results all the way out to someone is at higher risk. I don’t think I’ll be making recommendations on what bra someone should be wearing! Thanks to for sending me this one

THE EMOTIONAL LONELY CONFUSING JOURNEY WOTH NO PLAN

This is how a lot of athletes feel after ACL injury!

This study highlights three key features of the challenging time.

1. Emotional struggles in solitude
2. A journey with no map
3. A blurry beginning

We believe one of the key strengths of our Sports Knee Program is to do the exact opposite.

1. Support and guidance from peers and experts in your injury
2. A clear rehab plan with goals and targets
3. An accurate and clear diagnosis with next steps

More from the study:

The three overarching themes (Emotional struggles in solitude; A blurry beginning; A journey with no map) describe an emotional side of the knee injury that seems to receive scant attention by health care workers; this is compounded by an early care phase that is perceived as confusing, causing frustration and failing to set the knee-injured individuals up for the life-long journey with their knee injury.l

The void in the care pathway early after knee injuries means that non-professional, active individuals sustaining knee injuries are left alone in dealing with the wide-ranging consequences of their injury. Physios, despite all their good intentions, are seemingly unable to provide coherent care and may need more support and training in order to meet current care needs, including filling the early care void and taking a long-term perspective of future knee health. If knee-injured individuals are not sufficiently supported early, they risk entering a negative spiral of uninformed bad decisions regarding treatment, over-optimistic treatment expectations and a lack of awareness of long-term conse- quences of their knee injury, potentially missing out on important op- portunities to engage in activities to preserve future knee joint health

After a crazy few weeks of knee injuries across the AFL and NRL had the opportunity to shed some light on the injury to help increase understanding for the public.

“On rupture, an athlete will feel or hear a sharp pop when the ligament snaps. 
 
Intense pain in the centre of the knee will follow. Movement will exacerbate things.
 
From here most patients have three choices.
 
There’s surgical reconstruction.
 
There’s rehabilitation with delayed surgical reconstruction if instability continues.
 
There’s also the 12-week Cross Bracing protocolwhich has patients lock the injured knee at various angles to promote natural healing ligament.
 
Despite growing belief in non-surgical possibilities, it’s common practice for elite athletes to undergo surgical reconstruction as soon as possible to ensure they can complete rehabilitation and return to sport with a strong and stable knee quickly.
 
The hour-long operation usually involves taking graft tissue from another part of the body, typically the hamstring tendon, and using it to replace the injured ACL. This involves drilling tunnels in the bone to insert the new ACL before it is held into place bybuttons.
 
However, in the sub-elite population - and perhaps contrary to popular perception - latest research demonstrates that as many 50% of people can manage very well without reconstruction surgery……”

NEW GC KNEE GROUP PUBLICATION

This was a big project that took many years to complete and will certainly raise eyebrows in the Physio community for those treating total knee replacement patients.

Larissa headed the project with Adam and Adrian key members of the research team.

Snapshot……
The results of the KAPPA trial demonstrated that just over half of the included participants could successfully self-direct their rehabilitation following TKA without supervised physical therapy while also maintaining excellent clinical and self-reported outcomes.

This raises questions on who, when and how much Physio is needed after knee replacement. It is undoubtedly valuable in some but maybe less so in others.

Addressing these multifaceted factors is crucial to reducing the gender disparity in ACL injuries,” he tells Body+Soul “Research suggests that when opportunities, experiences, and training ages are equalised between genders, the observed differences in ACL injury rates certainly decrease reducing the influence of biological factors in ACL injury.

For those currently in play, the best form of prevention is the widespread implementation of ACL injury prevention programs, which of course comes down to funding, thus interest. 

“There are many programs that are well established across multiple sports, including the Perform+ by the Football Federation Australia and Prep to Play in Women’s AFL,” says Walker. 

“These programs have been shown to reduce the risk of ACL injury by 50 per cent as well as a whole host of benefits to performance and the reduction of other injuries such as ankle and hamstring injuries.” 
Not only could that work to improve players’ game, but save their careers. 

There’s no doubt that “widespread implementation will involve the significant increase in funding to help support the training of coaches, parents, and administrators at the club level to deliver the programs across all age levels,” Walker continues. 

“Further funding into research investigating ACL injury, including a national ACL registrar and support for the development and training of female athletes should also be a priority,”
