Alexo Disability Supports are urgently seeking an experienced support coordinator to join our rapidly growing Team, for an immediate start. This is an ongoing role that embraces a contemporary methodology and standard of practice while working flexibly in a work/home hybrid model, with most days working from home.
We are seeking an independent contractor (responsible for your own tax and super), who will work proactively with participants by coordinating and engaging supports to ensure they are getting the most out of their NDIS plans. This role gives you the opportunity to foster and build strong relationships with participants, their families, and carers. You will identify, connect, and negotiate with providers to deliver quality outcomes and timely service whilst working closely with other external stakeholders involved in an individual's care or provision of services.
Alexo Disability Supports is a registered provider of Support coordination (level 2 and 3), Recovery Coaching and Therapeutic Supports (currently social work and counselling). We have a reputation for providing excellent outcomes for our participants. We receive referrals from a variety of sources including child safety, LAC’s, complex pathways teams and our extensive allied health network. We cover the Logan, Ipswich, Redlands and Gold Coast Areas. We have an immediate caseload and many new referrals coming our way each week so you will be able to connect with a caseload immediately. Our office is based in Shailer Park, but you can be located anywhere within our catchment area. We have a CRM that is in use for case notes, billing, and time sheets.
To be successful in this role you will have:
• Minimum of 2 years of experience in a Support Coordination role.
• Understanding of the role of Support Coordination within the NDIS framework and the interplay with broader systems (health, education, child protection, mainstream supports)
• Preferable degree qualified
• Preferable child safety experience
• Be able to articulate a clear and ethical practice framework
You will also need exceptional skills in the following areas:
• Building rapport and relationships
• Well-developed communication and interpersonal skills
• Organisation and time management
• Ability to create and manage complex stakeholder groups
• Conflict Management
• Report writing
• Ability to work in a team and independently
In addition you will need the following:
• NDIS worker screening check clearance
• A Working with Children Check
• A current driver's license and well-maintained vehicle (with comprehensive insurance)
• Completed the NDIS Quality and Safeguards Commission online Worker Orientation training module.
• Membership with your relevant professional association
