Collagen is one of the most misunderstood topics in aesthetic medicine.

Topical collagen hydrates.
Oral collagen provides amino acids.

Both can have value.

But neither directly rebuilds the collagen framework within the dermis.

True collagen regeneration occurs when fibroblasts are stimulated through biological signalling - whether via prescription topicals, energy-based devices, microneedling, or other evidence-based treatments designed to trigger repair pathways.

This distinction matters.

Surface hydration improves how skin looks.
Structural stimulation influences how skin functions and ages.

At Plastic Surgery Queensland, we approach skin through the lens of anatomy, physiology, and long-term structural integrity - not trends or marketing claims.

Understanding the difference empowers you to make informed decisions about your skin.

If you’re considering collagen-focused treatments, a personalised consultation is the most reliable place to start.

Severe DIP Joint Osteoarthritis
When pain limits function, fusion can restore comfort and stability

This patient presented with severe osteoarthritis of the 2nd and 3rd distal interphalangeal (DIP) joints - the joints closest to the fingertips.

On the pre-operative X-ray, you can see:
• Advanced joint space narrowing
• Bone spurs (osteophytes)
• Irregular joint surfaces
• Joint deformity

These changes are typical of end-stage osteoarthritis and often cause:
– Constant aching pain
– Sharp pain with pinch or grip
– Swelling and stiffness
– Progressive deformity
– Reduced hand function

When non-surgical treatments (splinting, injections, anti-inflammatories) no longer provide relief, DIP joint fusion becomes a highly effective solution.

What Is DIP Joint Fusion?
Fusion (arthrodesis) removes the damaged joint surfaces and permanently joins the bones together in a functional position.

In the post-operative image, you can see:
• Internal fixation using compression screws
• Straightened, aligned joints
• Stable positioning to allow bone healing

Over time, the two bones heal into one solid bone - eliminating movement at that joint.

Why Fuse Instead of Replace?

At the DIP joint:
• Motion is minimal and not essential for overall hand functio
• Stability is more important than flexibility
• Joint replacements at this level have higher failure rates

Fusion provides:
✔ Predictable pain relief
✔ Correction of deformity
✔ Durable long-term outcome
✔ Improved pinch strength

Most patients do not notice any meaningful loss of function once healed.

Recovery Overview
• Protective splinting initially
• Light use within weeks
• Bone healing typically 6–8 weeks
• Gradual return to full hand function

The goal is simple: a straight, pain-free fingertip that functions reliably.

Educational Takeaway
Severe DIP joint osteoarthritis can be debilitating - but fusion surgery offers a definitive, stable, and highly successful solution when conservative care fails.

Pain relief. Alignment. Strength.
Restoring function at the fingertips matters.

Bilateral Breast Augmentation performed by Dr Theo Birch

Implant selection is a detailed and highly individualised process. During consultation, careful assessment is undertaken of breast width, existing breast volume, skin quality, and chest wall anatomy to guide appropriate implant choice.

Natural breast asymmetry is common, with many patients having differences in size or shape between the left and right breasts. In such cases, the use of different implant sizes may be considered to improve overall balance and proportion, rather than aiming for identical implants on both sides.

A history of significant weight loss is also an important factor in surgical planning. Weight changes can reduce breast volume and alter skin elasticity, which influences implant selection, positioning, and the overall surgical approach.

Important Do’s and Don’ts Before Plastic & Reconstructive Surgery

Proper pre-operative preparation is essential for patient safety, optimal surgical outcomes, and a smooth recovery. The following guidelines are designed to reduce surgical risks and support healing.

Pre-Operative DO’s ✔️

* Maintain Adequate Hydration
Ensure you are well hydrated in the days leading up to surgery, unless otherwise directed. Proper hydration supports circulation, kidney function, and the safe administration of anaesthesia.
* Follow a Balanced Diet
A diet rich in protein, vitamins, and minerals helps strengthen the immune system and promotes effective wound healing.
* Disclose All Medications and Supplements
Provide your surgeon with a complete and accurate list of:
- Prescription medications
- Over-the-counter medications
- Vitamins, herbal supplements, and injectables
This information is critical for surgical planning and anaesthetic safety.
* Inform Your Team About Weight-Loss Injections
If you are using weight-loss medications or injections (such as GLP-1 receptor agonists), it is essential to inform both your surgeon and anaesthetist. These medications can affect gastric emptying and anaesthesia safety and may need to be paused prior to surgery.

Plan Ahead for Recovery
Arrange transportation, post-operative care, and adequate time off work to ensure a safe and stress-free recovery period.

Pre-Operative DON’Ts ✖️

* Avoid Smoking and Ni****ne Use
Smoking, va**ng, and all ni****ne products significantly increase the risk of:
- Delayed wound healing
- Infection
- Poor scarring and tissue complications
Patients are typically advised to stop ni****ne use at least 2–4 weeks before surgery.
* Refrain From Alcohol Consumption
Alcohol can interfere with anaesthesia and increase bleeding risk.
* Do Not Take Certain Medications Unless Approved
Unless specifically instructed otherwise, avoid medications and supplements that may increase bleeding risk, including:
- Aspirin
- Non-steroidal anti-inflammatory drugs (NSAIDs)
- Certain herbal supplements
- Do Not Eat or Drink After the Prescribed Fasting Time
Strict adherence to fasting instructions is essential for anaesthetic safety.

GLP-1 medications have helped many patients achieve meaningful weight loss. For some, however, this change can leave behind excess or lax skin that doesn't fully retract - particularly around the abdomen, arms, thighs, breasts, and neck.

Excess skin isn't just a cosmetic concern, It can affect:
- Comfort and mobility.
- Skin health and hygiene.
- Clothing fit.
- Confidence and body proportion.

When should you see a Plastic and Reconstructive Surgeon?

Timing matters. A consultation is generally most appropriate when:
- Your weight has stabilised (often for 3-6 months)
- You're close to your long-term, maintainable weight
- Nutrition and protein intake are optimised
- GLP-1 dosing has plateaued or concluded

This allows for a more accurate assessment of skin quality, tissue redundancy, and surgical planning - and helps ensure results that are durable, balanced, and safe.

What surgery can address

Depending on the area and degree of excess skin, procedures may include;
- Abdominoplasty (tummy tuck)
- Arm or thigh lift
- Breast lift or reduction
- Body contouring after weight loss

At Plastic Surgery Queensland, post-weight-loss surgery is approached as reconstructive care - restoring structure, function, and proportion after a significant physical change.

Every body responds differently to weight loss. Our role is to guide you on if, when, and how surgery may fit into your journey.

Bilateral Breast Reduction performed by Dr Theo Birch

This 26-year-old patient underwent bilateral breast reduction surgery with Dr Birch, with the aim of reducing breast size and weight while creating a shape that better suits her body frame.

What many patients don’t realise:

• Breast reduction is about more than making breasts smaller — the remaining tissue is reshaped and supported
• Some patients notice temporary or permanent changes in ni**le sensation as nerves adjust
• The amount of breast tissue removed is planned around individual anatomy, not bra cup size
• Breasts can continue to change over time due to weight changes, hormones, pregnancy, or ageing
• As part of standard practice, removed breast tissue is sent for laboratory testing

About the surgery:

Breast reduction involves removing excess breast tissue, fat, and skin, then reshaping the breasts and repositioning the ni**le-areola complex. Incisions and scars vary depending on the technique used and the individual patient.

A consultation with a specialist plastic surgeon is important to determine whether breast reduction is suitable and to discuss risks, benefits, and alternative options.

Accessing Your Superannuation for Surgery - What Patients Should Know

Did you know that some patients may be eligible to access their superannuation early to help cover the cost of medically necessary surgery?

In Australia, this process is called early release of superannuation on compassionate grounds and is assessed and approved by the Australian Taxation Office (ATO) - not by your surgeon or clinic.

Who may be eligible?
You may be able to apply if your surgery is considered medically necessary to treat a condition that causes:
• Ongoing pain or discomfort
• Functional limitation
• Physical impairment
• Deformity related to trauma, injury, or a congenital condition

Cosmetic procedures alone are not eligible.

What’s involved in the application process?
To apply, you’ll typically need:
• A medical letter from your treating specialist or surgeon confirming the medical necessity of surgery
• A supporting letter from your GP
• Submission of an application through the ATO online portal (via myGov)

Important things to keep in mind:
• Approval decisions are made entirely by the ATO
• Your surgeon or clinic cannot guarantee approval
• Funds can only be released for approved medical purposes
• Processing times and outcomes vary between individuals

If you’re considering surgery and would like to understand whether early release of superannuation may be an option for you, this is something that can be discussed during your consultation.

Part 2 of our Your Questions Answered series on abdominoplasty, we're continuing the conversation by addressing some of the more nuanced questions around abdominoplasty (tummy tuck) surgery.

In this part, we discuss when abdominoplasty may be considered reconstructive rather than purely cosmetic, why posture plays an important role in surgical planning, why the abdomen can feel tight during recovery, and why achieving the tightest result isn’t always the safest or best goal.

These details matter when it comes to setting realistic expectations, optimising outcomes, and supporting long-term comfort and function.

If you missed Part 1, be sure to catch up.

Before & After: Bilateral Upper Blepharoplasty
Bilateral upper eyelid surgery performed by Dr Andrew Hadj.

What is upper blepharoplasty?
Upper blepharoplasty is a surgical procedure that involves the careful removal of excess skin - and in some cases fat - from the upper eyelids. It is commonly performed for patients experiencing eyelid hooding, heaviness, or skin redundancy that may affect appearance and, in selected cases, peripheral vision.

What this procedure aims to achieve:
• Improved upper eyelid contour and definition
• Reduction of excess or overhanging eyelid skin
• A refreshed appearance while preserving natural eyelid shape and function

Important considerations:
As with all surgical procedures, outcomes and recovery times vary between individuals. Swelling and bruising are part of the normal healing process, and results evolve over time. A thorough consultation is essential to assess suitability, discuss risks, benefits, and set realistic expectations.

Education and informed decision-making are key. If you’re considering eyelid surgery or would like to learn more about whether it may be appropriate for you, DM or call our office to schedule a private consultation (07) 3844 6069.

Skin Needling: What It Is, How Depth Matters, and When to Book Your Treatment

Skin needling, also known as collagen induction therapy, is an advanced treatment designed to stimulate the skin’s natural repair process. By creating controlled micro-channels in the skin, skin needling encourages collagen and elastin production - key components for healthy, resilient skin.

Why Depth Adjustment Is Important

One of the most important aspects of professional skin needling is depth selection. treatment depth is carefully adjusted based on:

- Skin concern (fine lines, acne scarring, pigmentation, texture)
- Treatment area (face, neck, décolletage)
- Skin thickness and sensitivity

Superficial depths target hydration, glow, and product absorption, while deeper settings are used to address concerns such as scarring and skin laxity. This customisation ensures effective results while prioritising skin safety.

What Skin Needling Can Improve

When performed correctly, skin needling may help to:
- Improve skin texture and tone
- Reduce the appearance of acne scars and fine lines
- Support collagen production
- Enhance overall skin quality and firmness

Results develop progressively as the skin rebuilds collagen over time.

When Is the Best Time to Have Skin Needling?

The ideal time for skin needling is typically during autumn and winter, when sun exposure is reduced. This helps protect the skin during the healing phase and supports optimal results. That said, skin needling can be performed year-round with appropriate sun protection and post-treatment care.

Most patients benefit from a series of treatments spaced 4-6 weeks apart, depending on their individual skin goals and concerns.

Why professional Treatment Matters
Medical-grade skin needling should always be performed by trained professionals who understand skin anatomy, depth control, and post-treatment care. This ensures safe treatment, consistent results, and minimal downtime.

At Plastic Surgery Queensland, skin needling treatment and tailored to each patient, focusing on long term skin health and natural results.

Why Early Treatment Matters in Hand Injuries

Hand injuries can affect more than just the skin. Beneath the surface are bones, joints, tendons and nerves that work together to provide movement, strength and sensation. Because these structures are closely connected, early assessment plays an important role in guiding care.

Delays in medical review may allow swelling, scarring or stiffness to develop. Over time, this can limit movement, reduce strength or affect fine motor control, particularly if deeper structures are involved.

Early assessment can help by:
• Identifying injuries that may require closer monitoring or specialist care
• Determining whether treatment is best managed conservatively or surgically
• Allowing timely referral to hand therapy when rehabilitation is needed

Recovery after a hand injury depends on many factors, including the type of injury, its severity, and individual health considerations. Early evaluation helps ensure treatment is tailored to the injury and supports safe healing over time.

🩺 If you’ve sustained a hand injury, a healthcare professional can help assess the injury and advise on appropriate next steps.
