In a breakthrough in detecting superficial endometriosis, the latest consensus statement from the IDEA group, co-authored by Prof George Condous, experts outline how ultrasound can be used to detect the signs of superficial endometriosis which has long been difficult to identify without surgery.

By describing the sonographic features and proposing a standardised scanning protocol, this paper brings us closer to earlier, less invasive diagnosis.

In an academic review co-authored by Prof George Condous drawing on 28 studies and data from 3 million women, researchers found that conditions like endometriosis and PCOS are linked to a higher risk of heart attack, stroke and other cardiovascular diseases. Inflammation and disrupted hormones could be the reason why, according to Prof Condous.

“Conditions like endometriosis and PCOS often involve chronic, low-grade inflammation,” he explains. “This can damage blood vessels and increase cardiovascular risk.”

SURGEON’S EYE VIEW | Negative sliding sign on ultrasound correlating with re**al deep endometriosis and complete pouch of Douglas obliteration at laparoscopy.

Ultrasound is a powerful tool not just for diagnosis, but also to guide surgical approach. One key test is the “sliding sign” which checks whether pelvic organs move freely in the pelvis. When organs are fixed or move together, it may indicate scar tissue from deep endometriosis.

Slides 2 and 3 show restricted mobility of pelvic organs – a negative sliding sign – and this was confirmed during laparoscopy in Slide 4, revealing re**al involvement and obliteration of the pouch of Douglas.

Cramps, nausea, headaches… periods can be uncomfortable, to say the least. A big part of this is due to hormone-like compounds called prostaglandins, which trigger uterine contractions to help shed the lining during your cycle.

In endometriosis, prostaglandins also affect lesions – the tissue similar to the uterine lining growing outside the uterus – which causes additional inflammation and pain. This is why anti inflammatory drugs, like ibuprofen, are commonly used to relieve its effects: they work by reducing prostaglandin production, easing cramps and other symptoms.

When it comes to endometriosis surgery, understanding the difference between excision and ablation matters.

Excision involves cutting out the lesion along with a small portion of surrounding tissue. These samples can then be sent for biopsy confirmation.

Ablation refers to the destruction of the lesion's surface using heat (e.g. diathermy or laser), without removing the tissue entirely.

At Sydney Endometriosis, excision is our preferred method. It’s considered the gold standard for treating deep infiltrating endometriosis, with lower recurrence rates, less risk of adhesions, and better outcomes for pain relief. By contrast, ablation is a more superficial approach and may carry a higher risk of scarring.
