1 in 7 Australian women will be diagnosed with breast cancer in their lifetime – but with regular screening and support, outcomes are improving every year.

🎉 Celebrating One Year of Telecare in Stanhope! 🎉

This week we proudly marked the first anniversary of the Stanhope Telecare Medical Centre – a milestone made possible through the partnership between Telecare, Kyabram District Health Service (KDHS), and the wonderful Stanhope community.

☀️ Guests enjoyed sunshine, games, a delicious BBQ, and even a special celebration for practice nurse Janine’s birthday!

A huge thank you to everyone who made this milestone possible. With your support, we look forward to many more years of providing healthcare shaped by the people it serves. 💙

𝐌𝐞𝐞𝐭 𝐋𝐨𝐫𝐞𝐭𝐭𝐚 – 𝐊𝐃𝐇𝐒 𝐀𝐝𝐯𝐚𝐧𝐜𝐞 𝐂𝐚𝐫𝐞 𝐏𝐥𝐚𝐧𝐧𝐢𝐧𝐠 𝐂𝐨𝐨𝐫𝐝𝐢𝐧𝐚𝐭𝐨𝐫

The Advance Care Planning (ACP) program commenced in 2012 for a 12 month trial to see if this is what our community wanted…and here we are still!
I was appointed the ACP Coordinator and still hold the position, however, I commenced at KDHS in 1980, in District Nursing/ Palliative Care and as a Breast Care Nurse (although I had some years off to have our children).

I love offering our community the opportunity to discuss and document their future health care wishes, if the day came and they could not speak for themselves. Knowing a person’s wishes, means they can be respected and have medical decisions made based on their values, beliefs and preferences.

𝐖𝐡𝐲 𝐝𝐨 𝐩𝐞𝐨𝐩𝐥𝐞 𝐰𝐚𝐧𝐭 𝐭𝐨 𝐭𝐚𝐤𝐞 𝐩𝐚𝐫𝐭 𝐢𝐧 𝐭𝐡𝐞 𝐩𝐫𝐨𝐠𝐫𝐚𝐦?
People really want to remove the burden of decision making from those they have appointed as Medical decision makers who are often family.
They want to have the peace of mind that the things that matter to them will be respected if they can’t tell us what’s important at the time.

𝐈 𝐨𝐟𝐭𝐞𝐧 𝐭𝐞𝐥𝐥 𝐭𝐡𝐞𝐦:
We have 100% mortality rate so Advance Care Planning is relevant to all of us.
And talking about dying, won’t make you die!

𝐍𝐞𝐰 𝐆𝐏 𝐬𝐞𝐫𝐯𝐢𝐜𝐞 𝐚𝐭 𝐒𝐭𝐚𝐧𝐡𝐨𝐩𝐞 𝐇𝐞𝐚𝐥𝐭𝐡

We’re excited to collaborate with Telecare to open Stanhope Telecare Medical Centre at Stanhope Health from Monday 12 August 2024 to help the community access high-quality, safe and reliable care.
The GP service will combine regular face-to-face consultations with weekly virtual telehealth consultations with Telecare GPs Dr Ken-Tze Koh and Dr Raymond Wen who have 20 combined years’ experience in rural health.
