Patellofemoral pain syndrome (PFPS) occurs around the patella, or kneecap and is often referred to as “runner’s knee” or “jumpers’ knee” due to its association with sports activities. Common among young adults (particularly females), PFPS can make everyday activities such as kneeling and stairclimbing difficult due to the associated pain and stiffness. Common symptoms include a gradual onset of dull, aching pain in the front of the knee, pain and popping or crackling sounds while climbing stairs or standing up. Though common among athletes, PFPS can also occur in nonathletes.

PFPS can develop due to misalignment of the kneecap as well as due to overuse during training. When PFPS occurs, nerves sense pain in the soft tissues and bone surrounding the kneecap. In some cases, PFPS will present with a condition called chondromalacia (softening and breakdown of the articular cartilage on the underside of the patella). While the damage to the cartilage cannot directly cause pain (no nerves in the articular cartilage), the damage can lead to inflammation of the synovium and pain in the underlying bone.

PFPS as a result of overuse can occur due to improper use of sports training equipment or techniques, or even due to something as simple as incorrect footwear or a change in sports playing surfaces.

PFPS as a result of patella misalignment can result from misalignment of the legs between the hips and ankles which causing the kneecap to shift too far to the left or right of the knee. Muscle weakness or imbalances especially in the quadriceps can also contribute to poor tracking of the kneecap within the trochlear groove.

Rotator Cuff tear vs strain. Which is worse?

The rotator cuff is a collection of muscles and tendons that surround the shoulder joint and retain the head of the upper arm bone firmly in the shoulder's shallow socket. Your rotator cuff comprises of four muscles: the subscapularis, teres minor, supraspinatus, and infraspinatus.

A rotator cuff tear is classified into 2 types:
- partial tear - some of your tendon is still attached to the bone. This type of tear is painful.
- complete tear - tendon is fully separated from the bone. This type of tear you might not feel any pain.

A rotator cuff strain is a stretching or micro-tearing of the tendons and muscles surrounding the shoulder joint, whereas a tear is a complete or partial tear of one or more tendons in the shoulder.

A tear can be happen due to a sudden injury causing a tear. A strain can happen overtime due to overuse and repetitive movements.

Treatment for a tear usually involves surgery to reattach the torn piece/s. A strain mainly involves conservative management with Physio. Physical therapy exercises to stretch and strengthen the shoulder joint while the micro tear heal itself.

Affecting approximately 13.6-20% of runners, most athletes have heard of somebody, or have themselves suffered from, the dreaded, ‘Shin Splints’. Known clinically as Medial Tibial Stress Syndrome, this condition is an overuse injury effecting the lower leg. It commonly presents as exercise-induced pain in the shin that decreases with rest (McClure & Oh, 2019).

Shin Splints is largely conservatively managed. Initial treatment will focus on pain relief, relative rest and icing. Following this, there can be a graded re-exposure to exercise. The correction of an individual’s biomechanics, taping and manual therapy should also be considered (Mayo Clinic, 2019).
