Another happy young patient. He is breathing and sleeping much better after treatment.
Generally we find that Enlarged tonsils and adenoids as well as nasal allergy can can block the upper airways leading to poor quality of sleep. This can then have a negative effect on learning, concentration an behavior. This little man had coblation “shaving” of his tonsils, adenoids and coblation shrinkage of the swollen lining of his nose.

Foreign bodies by definition are found in foreign places, such as the ear and the nose. This was a big red plastic button that ended up in this little guy’s right nasal passage. This has to be one the cutest and bravest little men I have met in a long time. I asked him how the button ended up in his nose ? He replied:”I pushed it in a little bit into my nose and my nose sucked it up”. This young man was very brave, allowing me to remove the button with a crocodile forceps. Ordinarily I use a wax hook to hook them out, but on this occasion, it kept spinning and slipping. Luckily I could grab it gently with a forceps and pull it out. Usually a forceps would not work with a hard slippery disc as one ends up pushing it deeper in the nose. I was fortunate to be able to crush an edge of the button and therefore to grip it.

A good history from the parent can be helpful. In this case Dad knew exactly what it was. He had a photo of it. This information can be helpful in planning the removal strategy.

Airway foreign bodies can pose problems depending where they are lodged. In the nose there is a danger of displacement and aspiration into the lower airways. Thus they pose a relative emergency more because they can move backwards and enter the lower airway and lungs causing breathing problems.

The type of foreign body and where it is lodged dictates how soon they need to be removed. Ordinarily any airway foreign body should be removed as soon as possible under controlled circumstances. Especially if these are in further down the upper aero digestive tract such as the oesophagus or trachea.

Disc or button batteries are very dangerous and can cause significant burns and erosion of soft tissue. These need to be removed urgently.

Vegetable matter, eg beans, in the ear should never be irrigated as they will swell when they absorb water. They require removal with suction or a small wax hook. Inert substances in the ear can safely be removed electively and do not pose an emergency unless there is an associated infection. Inert ear foreign bodies may be irrigated if they are small and not lodged tightly.

The safest way to clean ears of wax, foreign bodies or infection is with microsuction. This procedure involves the use of a microscope to magnify the ear and suction or a “vacuum cleaner”. This is one of my paediatric patients having his ears cleaned. This procedure is readily available through my rooms. It is safe and well tolerated even in young children. The procedure is especially beneficial in cleaning for chronic ear canal infections, otherwise known as “tropical ear or swimmer’s ear” or medically termed otitis externa. Microsuction is a better option to ear irrigation or ear syringing which involves flushing the ear with water. Ear irrigation with water can result in ear infections and can result in trauma to the delicate structures of the ear, especially the eardrum. The little guy in these pictures is a magnificent example of what we can achieve in the rooms. He is an absolute champion and is well on his way to recovery. Well done young man.

A Peritonsillar abscess or “Quinsy” is an abscess that occurs in the peritonsillar space. This space is between the tonsil and the pharyngeal constrictor.

It presents as a very painful throat, patients usually are unwell, cannot swallow due to pain and have difficulty opening their jaw.

They are found to have a swelling in the soft palate and the affected tonsil is displaced more centrally making it look larger than the other side. There is also swelling of the uvula.

Treatment involves evacuation of this pus. This is achieved either by aspiration using a large needle and syringe or incision and drainage. This is easily achieved in most adults under local anaesthetic. Antibiotics are also essential.

This abscess can recur in 20% of cases, either on the same side or the opposite side. It is recommended to have a tonsillectomy if the condition recurs.

This patient has a right peritonsillar abscess which was aspirated. About 7ml of pus was aspirated under local anaesthetic. Generally I tend to incise the area as well after aspiration to ensure no immediate local recurrence.
