Inclusion is not an added feature of support. It is a fundamental right that should be reflected in every NDIS plan.

True inclusion means creating supports that recognise each person’s identity, goals, and place within their community. It is not about fitting into a system, but about building support that fits around real life.

One of the most meaningful parts of support is seeing what can change when it is built around the person, not the system.

In one situation, an individual’s goal was simple. He wanted to live independently. Instead of applying a standard approach, the focus was placed on understanding what that independence would look like for him in real life.

With the right support, he was able to move into his own space and begin building stability. That sense of stability created something even more meaningful. He was able to reconnect with his daughter after many years and spend time with her in his own home.

Moments like these can lead to lasting change. They can support people to move away from harmful patterns and reconnect with what matters most in their lives.

Autism is not always visible, but that does not make it any less significant.

World Autism Awareness Day is an opportunity to focus on understanding, reducing barriers, and creating space for different ways of thinking, communicating, and experiencing the world.

Experiences such as burnout, masking, sensory challenges, and late diagnosis can all exist beneath the surface. Many people also find it difficult to explain their needs within structured systems like the NDIS.

Transgender Day of Visibility is a reminder that visibility is only meaningful when it is met with respect, understanding, and genuine inclusion.

For many transgender and gender diverse people, being seen does not always mean being supported. Creating environments where people feel safe, affirmed, and able to express their identity without fear is what truly matters.

Some of the most meaningful moments are often the simplest ones. A walk together, a genuine conversation, a shared laugh. These are the moments where connection grows, where people feel seen, and where space is created to slow down and simply be present. At Chosen Family, we believe support is not just about the tasks we complete, it is about how people feel in those moments and creating environments where individuals feel comfortable, confident, and truly themselves. Everyday experiences like these can build independence, strengthen relationships, and foster a deep sense of belonging, because the right support does not feel clinical or transactional, it feels natural, it feels safe, it feels human.

Capacity building skills are often built in everyday moments, like getting ready in the morning, planning a meal, or learning to cook safely at home.

In this video, our client shares that having kind, supportive workers around him has helped him with routines and cooking his own dinners.

To our incredible disability & mental health support workers at Chosen Family.

Thank you for the work that goes far beyond a shift checklist. Yes, there’s community access, personal care, appointments, and budgeting. But there’s also the emotional labour: holding space through frustration, supporting growth, celebrating wins, and walking beside people as they challenge themselves and reach their goals.

This work is deeply human. It’s care, connection, and dignity in action.

Last Friday reminded us of something deeply important: people thrive when they feel they belong.

We came together to celebrate the end of the year with familiar faces we know well, and new faces we were excited to welcome in. There was laughter, shared food, relaxed conversations, and that gentle festive buzz that only happens when people feel comfortable being themselves.

What stood out most wasn’t the decorations or the season, it was the sense of ease in the room. People reconnecting. People meeting for the first time. People showing up just as they are, knowing they’re welcome.

Community isn’t something that happens by accident. It’s built slowly, through trust, care, and making space for everyone to be seen and heard. Day's like this are a small but meaningful reminder of why that matters so much.

Grateful for everyone who joined us, brought their warmth, and helped make the evening what it was. Here’s to carrying that sense of connection forward into the year ahead 🎄✨
