🏖 Community Access: We facilitate connections and opportunities for social engagement.

🍝Meal preparation: We support your meal plans and look forward to supporting you prepare your meals

🛁Personal Care: Providing you with your personalised self-care plan

🏠 Daily Living Assistance: Support with daily tasks to enhance independence.

🏘 Short Term Respite Accommodation: A safe and comfortable space for a break.

🚗 Transportation, To assist you with your whereabouts. Any appointments at Anytime

HOSPITAL DISCHARGE SUPPORTS

🏥 Discharge from hospital can be planned or advised at short notice ahead of a weekend.

🫂Book a Companion/Care Giver for day/night support to allow you time to recuperate and regain your independence.

•Hospitals are keen to fast track discharge for various reasons. Returning to your home without additional support is less than ideal.

• Engage a Companion and schedule visitations
