This patient has achieved a natural-looking result with improved upper pole fullness, shape, and cleavage, while still maintaining softness and movement. We placed 370cc Mentor Moderate Plus implants, then used strategic fat grafting to refine contour, smooth transitions, and enhance overall harmony.

At 12 weeks, swelling has settled significantly and the implants are continuing to soften and “drop” into their final position, with scar maturation well underway.

When planning breast augmentation, implant position is just as important as implant size or shape. There are three main options:
1. Subglandular (on top of the muscle)
The implant sits directly behind the breast tissue and on top of the pectoralis muscle. This can offer a shorter recovery but may show more implant edges, particularly in patients with thinner tissue, and carries a higher risk of visible rippling.
2. Submuscular (fully under the muscle)
The implant is completely covered by the pectoralis muscle. This provides good upper-pole coverage but can limit lower-pole expansion and may be associated with animation deformity.
3. Dual plane – our preferred approach
With the dual plane technique, the upper portion of the implant is positioned under the muscle, while the lower portion sits beneath the breast tissue. This allows the implant to move naturally with the breast while maintaining muscle coverage where it matters most.
Why we favour dual plane placement
✔️ More natural breast shape and slope
✔️ Better soft-tissue coverage in the upper pole
✔️ Reduced risk of visible implant edges or rippling
✔️ Improved lower-pole fullness and projection
✔️ Allows tailored positioning based on anatomy and skin quality
As with all breast surgery, implant position is selected based on your individual anatomy, tissue characteristics, lifestyle and aesthetic goals. No single approach suits every patient, and careful surgical planning is key to achieving a balanced, long-lasting result.
