Endometriosis…. Is a complex and confusing disease that roughly 1 in 10 women of reproductive age will suffer from.

It presents in many different ways, but the two of the most common presentations are pelvic pain with your periods and infertility.

Many women will not find out that they have endometriosis until it comes time to conceive. Endometriosis can cause infertility in many ways and there is still a lot we don’t know about the condition. Some of the ways we think that it can impact fertility includes impacting egg quality, impacting implantation of the embryo as well as mechanical distortion of the egg and s***m meeting in the reproductive tract.

Management of endometriosis is complex… if you think endometriosis is impacting your fertility it is important that you seek a consultation from a qualified specialist.

What should I be eating to conceive? There is no trick to answering this one. Eating a balanced diet is important for conception. Ensuring you are having enough protein, carbohydrate, dairy, fruits and vegetables will put you in the best possible position physically and mentally to try and conceive. Common questions I often get are:

- Coffee
If you are anything like me you love your morning coffee! Well good news is, you can still have coffee if you are trying to conceive. But not an excessive amount. Studies indicate that excessive or large amounts of caffeine intake was associated with delays to conception. As such, a good general rule of thumb is 1-2 caffeinated drinks per day as a maximum and where possible go decaf!

- Alcohol
There is no data that is able to give us a safe amount of alcohol to drink in pregnancy and we do know that an excessive amount of alcohol can be harmful. As such, best practice is to cut alcohol when you are trying to conceive.

- Processed foods
Again, there is no comprehensive data about processed foods and conception. There are so many rules and regulations when you are trying to conceive and/or pregnant… I try not to get caught up on this one. Try to have a balanced healthy diet and minimise your processed foods, however if you are having the occasional ‘treat’ or ‘bad food day’, don’t beat yourself up about it.

This is a very personal choice and certainly not a question where ‘one size fits all’. The decision to freeze either eggs or embryos is largely dictated by your own personal circumstances.

The process of either freezing eggs or freezing embryos is very similar. A woman will undergo an IVF stimulation cycle. This is where she will inject herself with a medication to promote growth of as many follicles as possible; at the same time she will utilise another type of injection that prevents ovulation. As the follicles grow we will undertake regular blood tests and ultrasounds to determine the exact size and number of growing follicles. Once there is an optimal number and size, a third different injection is utilised to ‘trigger’ the cycle and the eggs are collected via a day procedure with a small needle on an ultrasound probe through the va**na.

If you are freezing eggs, this is where the process stops. The embryologist will have a look under the microscope and determine which eggs are mature versus not mature and all the mature eggs will be frozen at that point. If you go on to freeze embryos, it is at this point that we would obtain a s***m sample and combine s***m and egg together in the laboratory. Fertilization rates are checked at 18hrs or so after and then we grow the embryos for 5 days before freezing them. At this stage we refer to the embryo as a ‘blastocyst’.

Ovarian reserve is an area of assisted reproduction that has been heavily studied. We know ovarian reserve decreases with age. A woman is born with her lifetime supply of eggs at birth and as time goes on this number will progressively decrease. The rate at which it decreases is different for everyone. However, we do have some data and research that allows us to provide ranges of 'normal’ based on trends.

Two common ways of assessing ovarian reserve… or how many eggs you have left. Is by doing a pelvic ultrasound and assessing the number of follicles on the ovaries. Each follicle has the potential to house an egg for that month and can provide a rough indication on how the ovaries are aging. There is also a blood test that you can do, called AMH or ‘Anti-Mullerian Hormone’ this is the hormone secreted from the follicles that have the potential to produce eggs. Neither of these investigations are exact and can only provide a rough indication to both patient and clinician. It is important to remember that neither of these tests is a marker of your fertility or reflects your chance of falling pregnant. It also does not tell us about your egg quality…. Which is a very important component!
