Extract vs keep?
Occasionally we see cases which are exceptions to the rule. This female patient in her early 30s presented with an abscess around the lower incisors. Her general periodontal condition was diagnosed as stage III grade C. Periapical radiograph showed 31 had bone loss to the apex.

Despite the appearance of the tooth, conventional periodontal treatment was carried out, including subgingival debridement using ultrasonics and EMS perioflow. Oral hygine instructions was also carried out.

The interesting aspect of this case was the clinical and radiographic healing at 3 months following initial treatment. Bone fill was observed on the radiograph. Probing pockets depths have also reduced to a maintable level. No surgery, no fancy grafting was carried out. Just basic first principles of dentistry. Although the result is not 'cosmetic' the patient has the option to keep her teeth for at least a few more years
