Knee pain going downstairs is commonly caused by increased pressure behind the kneecap (the patellofemoral joint). When descending stairs the quadriceps muscles must control the body lowering with each step, which increases compression through the front of the knee. If the joint is irritated, weak or overloaded, this added stress can trigger pain around the kneecap during stair descent.
