When you review plans this January, check:
- Your goals are stated, with the support to reach them
- Hours and budgets are broken down so they’re easy to understand
- There’s a review process to update the plan as life changes
- They explain how progress and satisfaction will be measured
- And yes - ask each provider to walk you through their plan; how they explain it is as telling as what’s written
