HOW TO RELIEVE YOUR UPPER BACK PAIN

Upper back tightness is commonly seen in office workers and is a potential cause of upper back pain. Stretching it with this exercise can help reduce your discomfort as well as prevent it.

1. Make sure the band is on stretch from the beginning.
2. Retract your shoulder blade as you pull back.
3. You should feel the contraction in the middle of your shoulder blade

While we often focus on reducing pain and improving muscle and joint function in isolation early in our physio rehab, it is important to transition into "training the whole kinetic chain/ the whole system" as we transition back into sporting activities. This exercise is an example of training our ability to stay balance in athletic positions. Here's how -

(1) It loads your front leg preferentially - almost all sporting activities rely on our ability to load and push off of one leg at a time.

(2) Pushing Dumbbell up increases stability challenge - This exercise allows us to train increasing downward force, which as often required in landing during sprinting or after a jump, by bringing dumbbell overhead.

(3) Pushing dumbbell up with left arm increases stability challenge of right leg - it create a force pulling our torso away from our front leg. As a result, it increases the stability demand of our front leg's hip and core to keep ourselves upright.

Contrary to popular belief, physiotherapy rehabilitation doesn't always involve massages, stretching and theraband exercises! The hop step exercise shown here is a great example of an intermediate exercise for lower limb injury rehab to help you return to sport.

Let me explain how it applies to various rehab conditions:

(1) Lateral ankle injury - the side hop step allows you to challenge those injured ligament and muscles with higher speed but relatively safe load.

(2) Achilles injury - the forward-backward split step allows you to challenge the back leg by putting that Achilles under high speed loading, with front foot can help taking some of the load.

(3) Hip/ Knee injury - both stepping pattern serve as a sport-specific practice of proper hip-knee-ankle alignment in athletic positions.

You can progress this exercise by
(1) increasing step length
(2) increasing step speed (e.g. speeds per 30 seconds)
(3) increasing time (e.g. multiple rounds of 30-60 seconds)

Pro tip - Challenge yourself by performing multiple rounds of a set time (e.g. 30 seconds) and see if you are able to perform the same amount of steps within the time frame!

One of our favorite saying as Physiotherapist is "Motion is Lotion!" The cat-camel exercise shown here is a great example of this principle. This exercise is often given to new patient with stiff lower back or back pain that came on recently for these reasons:
(1) It allows you to re-introduce spinal motion in a safe manner since the spine is not under compression of gravity.
(2) Gentle motion also allows stretching and contracting back muscles.

Keys to perform the exercise
(1) Focus on pain free range
(2) Perform slowly and focus on moving each segment of your lower spine.
(3) Move your hip - tuck in and stick our your tailbone can enhance the effect of this exercise.
