Hi All,
We now provide home medication reviews, this service means that if you are taking 5 or more types of regular medications everyday you are now eligible for a yearly home medication review with an accredited pharmacist. It is a government funded program, so it is completely free of charge for you.
What is the benefit of the home medication review? The pharmacist can educate you on how to take your medications correctly, review any regular medications you are taking, check your at home techniques for inhalers/injections, Explain the advantages and disadvantages and can overall provide a medication review which provides a medication report for your GP to review for their opinion and suggestions.
How often can I use this service? You can request this service once a year, your GP can set up a reminder for next year so that you get an SMS for the next time you are due.
What is the cost? If you are eligible for this service, the service is completely FREE of charge for you as it is government funded.
How does it perform and what to expect? A Pharmacist will contact you to arrange a time suitable for you to do a home visit and they will come out to your home.

Dear all,

We would like to inform our patients that Claremont Meadows Medical Centre is in the transition to start using "AutoMed" online booking software platform to provide better and flexible online booking experience.
We are expecting that the new online booking system may launch in the next few weeks or so.
Our patients can still use the existing HotDoc platform till the end of February.
Please kindly download the " AMS Connect" App on your mobile to make sure you stay connected with your doctors and to continue receiving easy online booking experience.

Dear all,

We are very glad to inform you all that Claremont Meadows Medical Centre will change back to "Fully Bulk-billing clinic", effective from 1st November 2025 inclusive, with the government support to bear the cost.

Please note special procedures such as "iron infusion, implanon insertion, removal, biopsy, excision, etc" will still apply some gap fees. Otherwise, we are FULLY BULK-BILLING both weekdays and weekends!
