Supreme Community Care is an in home provider of disability & support services. We offer in home nursing and support services throughout South East Queensland.

Our services include Community Nurses for High Needs support, recovery, palliative care and more complex personal requirements. Clinical Mental Health supports for acute or continuing care, intervention supports and rehabilitation. Social Workers, Support Workers, Personal Carers and Domestic Assistants to assist with help around the house, personal care, independent living, social support to get out & about, getting you to and from appointments, respite care and companionship.
