Due to increasing demand ELPE Health have an urgent need to employ another Support Coordinator.
ELPE Health provide both Level 2 and level 3 Support coordination and offer a team of support coordinators to work with, and beside, to gain experience and new skills, and to bring your experience to the team to share.
You will be well supported and mentored by management, other SC's and our admin team.
We currently have 7 Support Coordinators with a broad range of skills and knowledge, with varied backgrounds, experience and understanding of sectors such as, Mental Health, Justice, Health (including hospital discharge), Employment and Child Safety Services Aged Care and the Disability sector.
We are offering a part time position minimum 30 hours a week, however this can be negotiated for the right candidate.
As has always been the case, the hours increase with case load.

The role of the Support Coordinator is to;
• Have experience in the sector, working knowledge NDIS and Coordination of Supports position and hold formal qualifications
Certificate 4 in Community Services (Disability) or equivalent or willing to work towards same.
• Support the implementation of the participants NDIS plan, assist them to understand the NDIS, navigate services and supports available to them.
• Strengthen the participants own abilities as able, building capacity and resilience to understand the NDIS and their plan scope.
• Identify strategies and solutions, monitor plan outcomes and plan expenditure.
• Negotiate with service providers to engage supports that enable participants to fulfil their goals, increase their own capacity and to participate more fully in the community
• Monitor the delivery of supports in accordance with NDIS, NDIS Quality and Safeguard Commission Principles and work within the expectations of the sector.
• Ability to coordinate assessments, implement supports as designated in the plan, and complete required documentation
• Ability to build and maintain working networks
• Demonstrate an outstanding work ethic and high degree of autonomy, self-motivation and professional judgement
• Ability to relate and communicate with diverse population and groups
• Hold and/or obtain a NDIS endorsed Working with Vulnerable Persons registration
• Hold a current Tasmanian Driver’s license
