Due to increasing demand ELPE Health have an urgent need to employ an additional Support Coordinator.
Background:
ELPE Health have been working with the NDIS SC for nearly 10 years and are an accredited organisation based in Ulverstone but working across the whole NW region.
ELPE Health provide both Level 2 and level 3 Support Coordination across a broad range of the sector and offer a team of experienced support coordinators to work with.
We currently have 7 Support Coordinators with a broad range of skills and knowledge, varied backgrounds, and experience.
Position:
We are offering a part time position minimum 37.5 hours a fortnight, however this can be negotiated for the right candidate.

All our staff are provided with mentoring, skills building and training opportunities.
The role of the Support Coordinator is to strengthen the participants abilities to coordinate and implement supports to enable them to participate independently in the community.
The coordinator must support the implementation of the participants NDIS plan, identify strategies and solutions as required, monitor plan outcomes and plan expenditure, build capacity and resilience in a confidential, caring and supportive manner. Assist in linking participants with appropriate identified mainstream services.
The person would be expected to work in collaboration with all the other support coordinators, allied health professionals, and administration team.
The applicant must
• Hold and/or obtain a NDIS endorsed Working with Vulnerable Persons registration
• Hold a current Tasmanian Driver’s license
Skills:
• Experience in complex plan and participant management
• Experience with supporting people with a disability, families, carers and networks
• Be able to plan, coordinate, manage and implement NDIS support packages including informal, mainstream and community funded supports in a complex service delivery environment
• Demonstrate ability to develop participant capacity and resilience to enable them to be informed, take an active role in their wellbeing, self-direct their own supports, networking, planning and implementation
• Be able to coordinate assessments, monitor and evaluate participants’ progress towards the plan and personal goals
• Assist participant to be confident, connected to their community and its services and maintain their independence
• Have a high level of communication skills demonstrated by the ability to develop, enhance and manage relationships with key stakeholders, participants, and participant networks
• Recognise diversity including disability, age, gender, culture, heritage, language, faith, sexual identity, relationship status, and other relevant factors from the participants’ background, and how it may affect their ability to participate in the community.
