The Restorative Care Pathway under Support at Home is designed to help older Australians recover strength and independence after illness, injury, or decline.

This targeted short-term program offers:
✔️ Up to $6,000 (or $12,000 for eligible participants)
✔️ Allied health and nursing support for up to 16 weeks
✔️ Goal-based care planning
✔️ A chance to avoid long-term care dependency
