We’re excited to welcome Tracy Sebenik, joining us our new Roster Coordinator at Sublime Care!

Tracy brings over 25 years of experience in the aged care sector, including more than 15 years specialising in rostering. She plays a key role in coordinating our participant schedules and matching clients with the most compatible support workers.

Her journey started as a Support Worker in 1998, before moving into community nursing, intake, and eventually rostering, where she found her love for scheduling and solving the “puzzle” of care coordination. Tracy also holds a Mastering the SCHADS Award certificate, adding to her wealth of knowledge in the sector.

Outside of work, Tracy enjoys puzzles, spending time with her family, and being a proud mum and grandma to her three daughters, son-in-law, and two grandchildren.

This week, we’re celebrating our incredible Speech Pathology team , the quiet achievers who help people find their voice, build confidence, and connect more deeply with the world around them.

Across Australia, around 1.2 million people need support with communication, and behind each of them is a story, and a team making a real difference. From early childhood through to older age, speech pathologists play a crucial role in supporting wellbeing, independence, inclusion, and opportunity.

Life and work feel more connected when the team culture is strong and the work truly matters.

Sinead joined our team just over two months ago and is already making a big difference.

As an Occupational Therapist, she supports older adults in staying safe and independent at home. Even small changes like home modifications or assistive equipment can have a huge impact.

She’s also found meaning in the day-to-day — a supportive team, flexible schedule, and work that feels genuinely fulfilling.

We’re excited to welcome Jake Yim to Sublime Care!

Jake supports older adults under the Commonwealth Home Support Programme (CHSP), working closely with clients to help them live safely and independently in their homes. He brings a person-centred, goal-focused approach, offering practical support through functional assessments, assistive technology, and home modifications.

With a Bachelor of Occupational Therapy from the University of Newcastle, Jake previously worked in occupational rehabilitation across CTP, Workers Compensation, and DVA schemes. His passion for aged care was inspired by his experience caring for his grandparents, and he now applies his clinical skills to support others in the community.

Outside of work, Jake enjoys playing indoor volleyball and basketball, staying active, and café-hopping in search of great coffee and local favourites.

Kaylan is one of the newest additions to our team and has already brought so much value through her clinical skills and passion for learning 🌈

As a Physiotherapist, she finds it incredibly rewarding to support people through their recovery. Whether it's helping them regain mobility after an injury or guiding them through more complex conditions, she brings genuine care and enthusiasm to every session.

She’s also sharing her knowledge through an inservice on Rheumatoid Arthritis, drawing from past experience and building on it for the benefit of the whole team.

What makes it all the more meaningful for her is the team culture. In Kaylan’s words, it’s a passionate and caring group of people working together with purpose 💙
