As you know Weipa Primary Health will close on November 15th. We want to inform our patients that we are currently only scheduling brief consultations renewing prescriptions and addressing symptoms that do not require further examination or investigating.
Patients who wish to have their medical files transferred need to complete a Transfer of Medical Records Request form. This form must be sent from their preferred Medical Practice. Once we receive the completed request, we will begin the process of transferring the medical records. This will help us to efficiently transfer the records to the new Practice.
