When should I see a Physiotherapist and when is Dr Google (or AI) enough?

We’ve all done it—typed symptoms into Google or asked AI for exercises. Sometimes, this can be useful. For example, if you’ve had a mild strain or a small ni**le and just want some general advice on rest, stretching, or gentle exercises, online information may point you in the right direction.

But here’s the thing—real life injuries aren’t always textbook.

⚠️ What looks like a “simple calf strain” might actually be a more serious tear, a nerve issue, or even a circulation problem. What feels like “just back pain” could be related to your hips, posture, or something else entirely.

This is where a Physiotherapist makes the difference.

A physio can:
🔹 Assess and diagnose the exact problem (not just the symptom).
🔹 Tailor treatment and exercises to your body, your lifestyle, and your goals.
🔹 Prevent the injury from recurring or getting worse.

𝐃𝐨 𝐲𝐨𝐮 𝐡𝐚𝐯𝐞 𝐧𝐨𝐢𝐬𝐲 𝐤𝐧𝐞𝐞𝐬?

Patellofemoral Pain Syndrome (PFPS), also known as kneecap mal-tracking, is a common knee condition characterised by pain around or behind the patella (kneecap). This pain is typically aggravated by activities that involve bending the knee, such as running, jumping, squatting, or kneeling. Crepitus, or a grinding noise is a common symptom.

PFPS can be caused by various factors, including:

1. Muscle Imbalances: Weakness or imbalance in the muscles around the knee, particularly the quadriceps and hamstrings.

2. Overuse: Excessive or repetitive activities that put stress on the knee joint, such as running long distances or participating in sports with frequent knee bending with not enough recovery time in between loading session.

3. Faulty Biomechanics: Issues with the alignment or movement of the patella, leading to increased stress on the knee joint.

4. Trauma: Direct injury to the knee such as a fall onto the kneecap.

Fortunately PFPS is one of the conditions that Physiotherapists treat with great success in most cases. For most people this will involve:

1. Taping may be used to unload the patella

2. Strengthening exercises for the muscles that hold the kneecap in place (inner quads) and gluteals to improve your biomechanics

3. Massage/releasing and stretching of tight structures that are pulling the patella out of alignment (the ITB and lateral quads).

I've hurt my lower back - should I get a scan???

The first episode of lower back pain (LBP) can be very worrying. You may have had friends or family needing back surgery or know people with chronic ongoing pain. And everyone seems to want to give you advice about what to do or who to see!

The good news is most back injuries are simple injuries no different to a sprained ankle. Statistics suggest that around 90% of first episodes of LBP will get better in 6 weeks. Your Regenerate Physiotherapist will help to guide you through the initial stages and then teach you to strengthen your back to avoid recurrence.

There is usually no need to have an X-ray or any type of scan in the first 6 weeks. In fact, research would suggest that having imaging done in the first 6 weeks can be detrimental to your recovery. For example, lets pretend you are a 50 year old male who has first time back pain and attends your GP who sends you for a CT scan. Naturally the CT scan will show some normal age related change. But, the way it is described on the report can make it sound like your back is falling apart and has all sorts of degeneration when in fact the result is very normal for your age. This can create stress, reduce activity levels, as you are scared of further hurting your back, and this inhibits your recovery as a result.

Current RACGP guidelines include the following statement:

"Unnecessary diagnostic imaging causes more harm than benefit because it can result in increased costs, delays in appropriate treatment, exposure to ionising radiation and increased absence from work. It may also lead to unnecessary referrals, procedures and surgery, and is associated with higher rates of prolonged disability".

There are of course exceptions. If a patient has significant neurological signs such as weakness, sensory loss, loss of reflexes or bladder changes we may do an MRI to look for nerve root compression or spinal cord issues. If a person with back pain has a history of cancer we may need to image to exclude metastasis. Fortunately these are rare and most episodes of mechanical low back pain will heal with the right advice, exercises and with no imaging needed.

We are very excited to welcome Sue Lin Kalisch to the Physiotherapy team at Regenerate. Sue Lin has over 20 years experience in Physiotherapy and Clinical Pilates both in Australia and London plus a stint with the Saudi Arabian Royal Family. She draws on her own rehab experience as a competitive swimmer along with experience in all football codes including the Western Reds (for those old enough to remember them!). She has worked with elite junior badminton and Masters athletics across an array of sports. In 2018 she completed her training in Vocal Physiotherapy for treating conditions of the speaking and singing voice and has a large following of musical theatre performers and singers. This had lead to a special interest also in the TMJ, chronic neck pain and headaches.

𝗜𝘀 𝗺𝘆 𝘀𝗵𝗼𝘂𝗹𝗱𝗲𝗿 𝗳𝗿𝗼𝘇𝗲𝗻?

The term "Frozen Shoulder" is bandied about a lot in lay terms but actually refers to a very specific diagnosis. A true frozen shoulder (medically called Adhesive Capsulitis) is a curious condition where the capsule of the shoulder joint becomes angry and inflammed often for no apparent reason. It is most common in females aged 40-60 years and can commence following mild trauma but often with no obvious cause or injury. We also see a variation called a post-operative frozen shoulder with patients who have been in a sling for an extended period following shoulder or elbow surgery.

It is often tricky to diagnose in the early stages as the shoulder becomes painful but often does not actually start to "freeze" (stiffen up) until a few months down the track. And it can be a real spectrum in terms of the degree of pain and disability with some cases very mild and others with significant pain and restriction. The condition can last for between 6 months and 2 years. Some people get full resolution and some are left with some mild stiffness.

Physiotherapy treatment for milder frozen or stiff shoulders involves soft tissue releases, joint mobilisation and specific exercises and this can be very effective. With a full blown frozen shoulder there is little we can do as Physiotherapists from a "hands-on" perspective but we will give you exercises to do to try and keep your shoulder as mobile as possible. There is some evidence for cortico-steroid injections to help with the pain if required.

Regenerate are thrilled to welcome our new Remedial Massage Therapist Catherine O'Rourke from Exhale RM to the massage team. Catherine is a highly qualified and dedicated Remedial Massage Therapist with expertise in a variety of techniques including deep pressure, remedial, myo-fascial cupping, sports, pregnancy massage and relaxation. Specialising in the treatment of upper back, shoulder and neck pain Catherine takes a personalised approach to care. She collaborates closely with clients to understand their specific needs and preferences, ensuring tailored treatments that deliver the desired outcomes.

My back hurts - should I get a scan?

A significant episode of lower back pain (LBP) can be very worrying. You may have had friends or family needing back surgery or people having chronic ongoing pain. And everyone seems to want to give you advice about what to do or who to see!

The good news is most back injuries are simple injuries no different to a sprained ankle. Statistics suggest that around 90% of first episodes of LBP will get better in 6 weeks. Your Physiotherapist can help to guide you through the initial stages and then teach you to strengthen your back to avoid recurrence. 𝐗𝐫𝐚𝐲’𝐬 𝐨𝐫 𝐨𝐭𝐡𝐞𝐫 𝐬𝐜𝐚𝐧𝐬 𝐬𝐮𝐜𝐡 𝐚𝐬 𝐌𝐑𝐈 𝐚𝐫𝐞 𝐧𝐨𝐭 𝐧𝐞𝐞𝐝𝐞𝐝 𝐢𝐧 𝐭𝐡𝐞 𝐟𝐢𝐫𝐬𝐭 𝟔 𝐰𝐞𝐞𝐤𝐬 unless you have loss of feeling and power in one/both legs indicating a significant nerve impingement.

Recovery from LBP is rarely linear and you may have some hiccups along the way. While 6 weeks is the average some people feel 100% within two weeks and other can take months if the injury is more significant or if it affects a slower healing structure such as an intervertebral disc.

Physiotherapy plays a crucial role in the recovery of Low Back Pain. In the early stages "hands on" techniques can provide pain relief and improvements in movement. Your Physiotherapist will provide exercises to do in the early stages plus strengthening exercises as you progress.
