Due to recent growth, CnC Disability Support Services has an exciting opportunity available to join our small, highly collaborative team of Support Coordinators. The role will initially be on a part time basis delivering high-quality support coordination services for new clients as well as a case load of existing NDIS participants.
As an NDIS Support Coordinator, you will manage your portfolio of NDIS participants relying on your ability to build rapport and foster relationships. You will use your high-level stakeholder management skills to build and leverage networks to ensure the best support for your participants. You will set up service agreements with providers, prepare plan reviews, conduct risk assessments and provide support should critical issues arise.
The successful candidate will need:
• Previous experience as a Support Coordinator - or a similar role in disability or community services
• NDIS Worker Screening Clearance & Working with Children Check
• Driver's license and reliable transportation
• Qualifications in Disability, Aged Care or Allied Health
• Excellent communication and interpersonal skills
• Empathetic attitude and a genuine passion for the disability industry
