Understanding Thoracic Outlet Syndrome (TOS)

TOS refers to a group of conditions that results from compression of the neurovascular structures that pass through the thoracic outlet.

Neurogenic TOS (95%) -> Pain, tingling, numbness, pins & needles, and/or weakness in the distribution of affected nerve roots

Vascular TOS (~5%) -> Upper extremity swelling, venous engorgement, discoloration, feelings of arm heaviness, and pain

Importance of Diagnosing TOS Accurately

TOS can be easily misdiagnosed due to the vague nature of the symptoms, however, the management/ treatment approach can be completely different. It's crucial to rule out other conditions through a thorough evaluation by a physiotherapist:

✔️History
✔️Symptoms (Nuero vs vascular)
✔️Physical Tests (Muscle & Nerve)
✔️Postural Assessment
✔️Special Tests
✔️Imaging (X-ray, CT, MRI) if needed

❗❗❗Early and accurate diagnosis of TOS, especially neurogenic TOS, is crucial for achieving optimal outcomes and minimising complications. By receiving early treatment, it shortens the course of the condition and prevents permanent/ long-term pain or loss of motor function.

Multidirectional instability (MDI) of the shoulder

➡️ MDI involves injury of the structures which hold the humeral head in the centre of the socket. This causes an increase in translatory movement that interferes with function and can produce pain with movement
➡️ MDI can present as recurrent dislocation, recurrent subluxation (partial dislocation) or pain due to recurrent loss of control with movement
➡️ MDI involves at least two directions of movement: anterior (forwards), inferior (downwards) or posterior (backwards)

Assessment of Multidirectional instability (MDI) of the shoulder

➡️ Can be assessed by your physiotherapist who will check the stability in different directions of shoulder movement: anterior (forwards), posterior (backwards) and inferior (downwards)
➡️ They will also check your muscular control, strength and the biomechanics of your movement
➡️ It may be necessary to get an MRI scan to check which structures have been injured especially in the event of dislocation/s

Does your squat look like this? and what does it mean?

Knee valgus = knees collapsing inwards during the squat.

🙁 It may indicate weaknesses in hip abductors (external rotation), or could be due to a lack of hip internal rotation range. I
🙁 It can lead to increased stress on the knee joint, potentially causing pain or injury.
🙁 It can limit your ability to get full squat depth

💡To address this we want to exercise targeting hip stability and range as well as address proper squat mechanics.

𝗡𝗮𝘃𝗶𝗰𝘂𝗹𝗮𝗿 𝗣𝗮𝗶𝗻

Navicular pain or stress fractures are the nemesis of any runner and can spell an end to a season if not addressed appropriately.

The reasons for their occurrence are largely related to running load and biomechanics, but biomechanics themselves are multifactorial and can include any of the following:
High arches or excessively rigid foot (inability to pronate/unload the foot)
Weakness in the hip adductors and antero-medial chain
Developmental abnormality

We utilize the Blackboard in these cases in some of the following ways:

Video 1 - Mobilising the whole foot to encourage pronation
Video 2 - Encouraging pronation control on loading
Video 3 - Encouraging active use of lateral foot upon push off
