When managing large uterine fibroids preoperative planning is essential.
The patient must be optimised so their haemoglobin is at a stable level, diet presurgery is protein rich and intraoperatively reduction of blood loss by use of advanced bipolar instruments which seal vessels and avoidance of sutures.
Post-operatively adequate analgesia early mobilisation and diet encourage a quicker recovery.
These fibroids are HUGE!!

What you see in the yellow circle is abnormal dense endometriosis deposit on tissue which connects to the bladder (green) and cervix.
This patient had urinary symptoms and a pulling sensation after urination.
Multidisclipinary input from a urologist was important as a cystoscopy was normal.
After excision the tissue was not under tension and the pulling pain had resolved.
As this case was done laparoscopically the patient had a speedy recovery amd was able to return to normal activities earlier than if this was done via a traditional open technique.

What you see here is an ovarian torsion where a cyst around 15 cm had twisted and became infected
This procedure was done laparoscopically and shows the cyst which is 3 cm below the belly button and is adherent to the lower pelvic side walls 1st slide..
Before anything could be removed all the structures had to be identified prior.
The torsion is depicted in the 2nd slide and the blue black structure is the ischaemic fallopian tube and o***y, the last slide is with the o***y removed
