我們可以選擇不虛弱 : 肌少症常被忽略,背後的關鍵原因更常被忽略

家裡的長輩如果最近常說:「怎麼越來越沒力」、「以前爬樓梯都沒問題,現在才兩層就喘得要命,手腳還會抖」,這些看似平常的抱怨,其實可能不是單純的「老了」。
這很可能是肌肉正在流失的警訊,肌少症。

當我們聽到這些話,不妨先停下來想一想:是不是該幫家人評估一下,肌肉是不是悄悄出現問題了?
因為肌少症是可以預防、可以調整的,如果及早發現,就還來得及扭轉下坡路。

肌少症不是老年病,是生活出現漏洞的提醒

根據2025年《Clinical Interventions in Aging》期刊的研究回顧指出,全球60歲以上的長者當中,大約每三人就有一人正在面對肌肉流失的問題。而在安養機構裡,這個比例甚至可能超過一半,情況不容忽視。

但我們真正該關心的,不只是這些冰冷的數字,而是它背後的生活真相,走不遠、站不久、力氣漸弱、跌倒頻率增加、甚至連生活自理都開始吃力。很多人誤以為這就是「正常老化」,其實這些變化,很可能是肌少症正在默默發生。

肌少症就像一艘正在漏水的小船,並不是突然沉沒,而是每天一點一滴地失去支撐力。幸好,這艘船是可以修補的。只要我們及早察覺、積極介入,不只是延緩,它甚至可以被逆轉。這是我們能夠掌握、而且值得投入的健康轉折點。

肌肉在消失,常常不只是因為年紀,而是這幾件事沒顧好 : 

老化
50歲之後,肌肉每年平均會流失1–2%。特別是掌握瞬間反應和支撐力的快縮肌最先流失。當這些肌肉不見了,就連「踩煞車」、「閃跌倒」、「爬樓梯」這些基本動作都會變得困難。

慢性發炎
我們以為自己沒生病,其實身體正默默「著火」。像TNF-α、IL-6這類發炎物質會破壞肌肉、加速蛋白質分解。高血糖、壓力、熬夜、不運動,都是讓這場「內部小火」一直燒的燃料。

粒線體功能障礙
粒線體是肌肉的「發電廠」。年紀一大、壓力一多,它就可能罷工,導致能量不足、自由基累積,肌肉自然無法持續運作。結果就是越來越容易累、反應變慢。

荷爾蒙變化
維持肌肉的生長與修復需要各種荷爾蒙幫忙:像是生長激素、睪固酮、甲狀腺素,甚至維生素D都很重要。這些訊號一少,肌肉就像工地沒人指揮一樣,放空、停工、退化。

吃太少又動太少
年紀大了怕胖、怕脹氣、怕不好消化,很多人變得吃得少、吃得單調。蛋白質不足,肌肉沒有原料,當然會變少;活動量少、長時間久坐,肌肉更是「有閒就退休」。

這些風險每天都在我們身邊打轉,而最令人振奮的是,我們可以改變它們。

一口蛋、一杯優格、一點陽光,都是養肌的開始

我們常說「肌肉是吃出來的」,這不是玩笑話。
對中高齡者來說,每天每公斤體重建議攝取 1.2~1.6克蛋白質,這遠比一般人想像的多。一顆蛋、一杯豆漿、一碗雞肉粥,都是累積肌肉的基礎。
如果牙齒不好,別擔心,可以改喝乳清蛋白,好吸收又輕鬆補。

還有維生素D,不只骨頭,肌肉也超需要。每天曬10~15分鐘太陽,或補充800~1000 IU的D3,就像替身體打開一盞「肌肉再生的開關」。

如果是慢性發炎或粒線體衰退,肌肉還有機會被救回來

很多人一聽到「慢性發炎」或「粒線體功能變差」,會以為這是無法逆轉的生理老化,但其實不然。我們現在知道,發炎體質是可以調整的,而粒線體的效率也能透過一些方法重新被啟動。

像是調整飲食、補充抗氧化抗發炎營養素(如Omega-3、薑黃素、輔酶Q10、綠茶多酚)、適量運動、良好睡眠,甚至只是每天散步曬10分鐘太陽,都能幫助降低體內的發炎反應,也能喚醒粒線體的活力。

這些日常的小選擇,就像是慢慢幫身體「退燒」的過程。發炎少了,粒線體的壓力減輕,肌肉就會比較願意生長、不容易被分解。也就是說,從調整體質開始,肌肉還有機會被救回來。

運動不是選擇,而是生活必需品

很多人聽到運動就先搖頭:「膝蓋不好啊」「我沒體力」……但其實,運動不一定要跑步、跳舞或舉重。

坐在椅子上也可以鍛鍊:椅子站坐、牆壁深蹲、彈力帶拉伸,這些都是非常有效的阻力訓練。

目標不是要變成虎背熊腰的運動員,而是要讓身體記得「我還在用」。就像電池需要常常充電,肌肉也需要動一動來提醒它活著。

腸道菌和牙齒,也能幫助您「長肌肉」

這幾年的研究發現,腸道菌相也跟肌肉健康有關。好菌會製造短鏈脂肪酸(SCFAs),幫助肌肉合成和抗發炎。

每天吃點高纖蔬菜、水果、一杯優格,或偶爾補點益生菌,這不只是「顧腸胃」,也是在「顧肌肉」。

還有一個常被忽略的重點:牙齒。咬不動、吃不好,很多人蛋白質自然攝取不足。記得定期看牙醫,平常也可以練咀嚼、訓練口腔肌力,這些對長肌肉很關鍵。

我們不追求逆齡,但我們可以選擇不虛弱

肌少症其實是一種提醒,一個讓我們重新思考:
「我還能做些什麼,來把自己的生活撐起來?」

不需要等到不能走、不能提、不能自理才開始行動。
從今天起,多吃一點營養、多動五分鐘、多走兩步、多睡一點覺,這些小小的習慣,就是我們對抗肌少症、留住行動力的最好選擇。

Glad that I can help you 💕

Uncle had suffered from low back pain and knee pain after sitting for long hours for gardening.

And he was completely free from pain the next day after the treatment

(This is our seniors problem: is either they don't move, or they can't stop their work 😆😆)

After assessment, I found out that he was actually having ITB tightness and piriformis tightness due to prolong sitting and it caused low back pain.

What have I done for his condition?
1. Infrared to relax the muscles
2. Soft tissue manipulation on the targetted muscles
3. Stretch on the tight muscles
4. Strengthen the weak muscles
5. Design a customised home programme exercises for him

So I asked him after 5 is? what is this?
( at the same time i showed my thumb 👍 as a sign of "6"),

"Good!" he said..

Hahahhaha.. I laughed and looked at him..
He was so cute..

He wasn't wrong, it can be GOOD 👍 as he did well that day.

1..2..3..4..5.......他停了下来

于是我问他5之后是什么? 这是什么?
(同时我伸出大拇指👍表示“6”),

“很好!” 他说..

哈哈哈哈。。我大笑,看着他。
他太可爱了..

他也没有错,这可以是“很好👍”,
因为他那天表现很好。

Uncle was having knee OA and gout for long time. His knee pain worsen and affected his walking since July'2023.

He started treatment with me for 1 month, 3x a week, his pain reduced, only gout attacked intermittently.

This morning, his message made my day. Happy to see that he is able to resume his morning walk and his favourite gardening .

👍👍 for uncle for his determination in his physio sessions.
