New evidence is strengthening the link between periodontitis and atrial fibrillation. Researchers report that chronic gum inflammation may increase the likelihood of developing atrial fibrillation and worsen outcomes in patients already diagnosed.

Persistent periodontal infection appears to drive systemic inflammation, immune activation, and atrial fibrosis. These biological changes may create the conditions for abnormal electrical activity within the heart.

The review highlights that periodontal treatment, including professional scaling and improved oral hygiene, significantly reduces inflammatory markers associated with atrial fibrillation risk.

Experts propose that oral health should be incorporated into integrated cardiac care, as managing gum disease may become a key strategy in preventing and controlling atrial fibrillation.

নিরবচ্ছিন্ন পেরিওডন্টাল সংক্রমণ শরীরে সার্বিক প্রদাহ, রোগ প্রতিরোধ ব্যবস্থার সক্রিয়তা বৃদ্ধি এবং হৃদপিণ্ডের অ্যাট্রিয়ামে ফাইব্রোসিস সৃষ্টি করতে পারে। এসব পরিবর্তন হৃদপিণ্ডে বৈদ্যুতিক সংকেত চলাচলে অস্বাভাবিকতা তৈরির জন্য উপযুক্ত পরিবেশ তৈরি করে।

A historic decision has been made in Geneva.
More than 150 countries, under the Minamata Convention on Mercury, have agreed to completely phase out mercury-based dental amalgam by 2034 — a landmark step in global health and environmental protection.

Mercury, once a mainstay of dental restorations for over 175 years, is now officially recognized by the World Health Organization as one of the top 10 chemicals of major public health concern — toxic to the nervous, immune, and renal systems, and a pollutant that persists in nature.

The agreement sets a science-based, time-bound goal to eliminate mercury in dentistry, replacing it with safer, modern alternatives such as composite resins, glass ionomers, and ceramic restorations. It marks a decisive shift toward mercury-free oral care and a safer, more sustainable dental future.

While some nations pushed for an earlier 2030 ban, consensus was reached for 2034 — balancing global access, affordability, and readiness of alternatives. The move follows growing international bans on mercury in batteries, cosmetics, and medical devices.

A case of MUCOSAL MELANOMA in a 48 year-old male patient.

Mucosal melanomas are very rare highly aggressive tumors. They are a distinct biological entity that varies from cutaneous melanomas.

Mucosal melanoma has a tendency for early nodal and distant metastasis. Surgical resection remains the main mode of treatment. Adjuvant radiotherapy improves local control but not survival. Targeted therapy and immunotherapy appear to be promising in a few number of cases.

The 5-year overall survival rate ranges from 20% to 50%.

Notice the palatal extension of the lesion.

৪৮ বছর বয়সী এক পুরুষ রোগীর মিউকোজাল মেলানোমা (Mucosal Melanoma)-এর একটি কেস।

মিউকোজাল মেলানোমা অত্যন্ত বিরল এবং অতি আক্রমণাত্মক (highly aggressive) ধরনের টিউমার।
এটি ত্বকের মেলানোমা (cutaneous melanoma) থেকে জৈবিকভাবে আলাদা একটি রোগ।

এই ক্যান্সারের বৈশিষ্ট্য হলো— এটি খুব দ্রুত লিম্ফ নোড (nodal) এবং দূরবর্তী অঙ্গে (distant) ছড়িয়ে পড়ে (metastasis)।

🔹 চিকিৎসা
• অস্ত্রোপচার (Surgical resection) এখনো পর্যন্ত প্রধান চিকিৎসা পদ্ধতি।
• সহায়ক রেডিওথেরাপি (Adjuvant radiotherapy) স্থানীয়ভাবে রোগ নিয়ন্ত্রণে সহায়তা করে,
তবে এটি রোগীর মোট বেঁচে থাকার হার (overall survival) বাড়াতে পারে না।
• টার্গেটেড থেরাপি (Targeted therapy) এবং ইমিউনোথেরাপি (Immunotherapy) কিছু রোগীর ক্ষেত্রে আশাব্যঞ্জক ফলাফল দেখিয়েছে।

🔹 রোগের পূর্বাভাস

এই রোগের ৫ বছরের মোট বেঁচে থাকার হার (5-year overall survival rate) প্রায় ২০% থেকে ৫০% এর মধ্যে থাকে।

🔹 বিশেষ লক্ষণ

ছবিতে দেখা যায় যে ক্ষতটি তালু (palate) পর্যন্ত ছড়িয়ে গেছে।

🦷 মাড়ির রোগ (Gum Disease) দুই ধাপে হয়:

১. Gingivitis (মাড়ির প্রদাহ)

👉 মাড়ি লাল হয়ে যায়, ফোলাভাব আসে এবং ব্রাশ করলে রক্ত পড়ে।
👉 এটা মাড়ির রোগের প্রথম ধাপ — সঠিকভাবে যত্ন নিলে সহজেই ভালো হয়।

২. Periodontitis (গভীর মাড়ির রোগ)

👉 যদি Gingivitis-এর চিকিৎসা না করা হয়, তাহলে দাঁতের গোড়া নড়ে যায়, হাড় ক্ষয় হতে শুরু করে, এবং দাঁত পড়ে যাওয়ার ঝুঁকি তৈরি হয়।

• দাঁত ব্রাশ করার সময় রক্ত পড়া
• মাড়ি ফুলে যাওয়া বা লাল হয়ে যাওয়া

• দাঁত নড়বড়ে হয়ে যাওয়া

🧼 ১. ঘরোয়া যত্ন:
• দিনে দুইবার সঠিকভাবে দাঁত ব্রাশ করুন (soft brush ব্যবহার করুন)
• প্রতিদিন ফ্লস করুন (dental floss)
• মাউথওয়াশ ব্যবহার করুন (যেমন Chlorhexidine)
• লবণ পানিতে কুলকুচি করুন (প্রাকৃতিক জীবাণুনাশক)

🦷 ২. ডেন্টাল চিকিৎসা:
• Scaling and Root Planing: ডেন্টিস্ট মাড়ির নিচ থেকে প্লাক ও টার্টার পরিষ্কার করেন।
• Antibiotics: কোনো কোনো ক্ষেত্রে ইনফেকশন থাকলে ডেন্টিস্ট ওষুধ দিতে পারেন।
• Surgery: মারাত্মক ক্ষেত্রে ফ্ল্যাপ সার্জারি বা গ্রাফটিং করতে হতে পারে।
