শেফা পয়েন্ট।

প্রতি শুক্রবার :
সকাল ৯ টা থেকে বিকেল ৪ টা
মেডিল্যাব (ক্লিনিক),
চৌদ্দগ্রাম, কুমিল্লা।

সবাইকে আল্লাহ পাক সুস্থ রাখুক এবং রমাদান এর পূর্ণ ফায়দা আদায় ও আমলের তাওফিক দান করুন, আমীন।

Hypothyroidism is now a silent disease running all over in the society..
Some of them are being undiagnosed for prolonged duration...
Some are diagnosed incidentally when they accompany their patients at chamber..

Be aware of

Unexplained weight gain
Undue fatiguability
Hypersomnia
Change in Menstruation Cycle(in female)
Forgetfulness
Somehow Constipation
Anhedonia

Dont take all these complaints as a normal phenomenon....
Many a cases i did find out Alhamdulillah....
( Hashimoto's Thyroiditis ) -mostly among them got finally after checking Antibodies and Endemic cause is nearly zero now due to negligible requirement of Iodine is needed for an adult daily..

They can easily take 150 microgram of Iodine per week having fortified iodised salt or sea food now a days..

পর্ব-২ (ডেংগুর আদ্যোপান্ত)
১ম পর্বে বলেছি প্রাথমিক ভাবে কি করবেন,,

যখন রক্ত পরীক্ষায় আপ নার ডেংগু হয়েছে বিষয়টি পরিষ্কার হবেন,তখন কিছু করণীয়।

---শুধুমাত্র প্যারাসিটামল জাতীয় ওষুধ সেবন করবেন জর কমানোর জন্য, সাথে স্পঞ্জিং করতে পারেন।
---কখনোই ব্যথানাশক ডাইক্লোফেনাক ওষুধ অথবা সাপোজিটরি ব্যবহার করবেন না।
এতে হেমোরেজ এর ঝুঁকি আরো বাড়ে।
----প্রচুর পানি,স্যালাইন, জুস(হোম মেইড), স্যুপ এবং ভিটামিন-সি যুক্ত খাবার খান।

জর আসার ৩-৫ দিনের মধ্যে Dengue NS1 টেস্ট করাবেন,
CBC এবং SGPT সহ।
৫ দিন পার হয়ে গেলে IgM for Dengue করাবেন।
IgG ও সাথেই করা হয়, এটা আপনার পুর্বের ডেংগুর উপস্থিতি প্রমাণ করে।

পর্বঃ ১(ডেংগুর আদ্যোপান্ত)

বর্ষা মৌসুমের শেষে এক আতংকের নাম ডেংগু।
পরিত্যক্ত পাত্র,ফুলের টব,এসির নিঃসরিত পানি, টায়ার এ জমে থাকা পরিষ্কার পানিতেই ডেংগু ভাইরাস বহনকারী এডিস মশা ডিম পারে,,
সেখান থেকে লক্ষ লক্ষ এডিস মশা জন্মগ্রহণ করে।
ডেংগু ভাইরাসের প্রজাতি এ যাবত ৪ টি আবিষ্কৃত হয়েছে।
ডেন-১, ডেন-২, ডেন-৩, ডেন-৪।

এ বছর ডেন-৩ এবং ডেন-৪ এর প্রভাব বেশি লক্ষণীয়।
দেখা যায়,যারা পুর্বে ডেন-১ অথবা ডেন-২ দিয়ে আক্রান্ত হয়েছিলেন, তারা এবার ৩ এবং ৪ দিয়ে আক্রান্ত হচ্ছেন,
যেটা হেমোরেজিক ডেংগু জরের প্রধান কারণ।

যখনই উচ্চ মাত্রার জর আসবে এবং এর সাথে তীব্র মাংসপেশীর ব্যথা,চোখের কোটরে ব্যথা,মাথা ব্যথা এসব উপসর্গ থাকবে,
দেরি না করে CBC এবং Dengue NSI করিয়ে নিন,,
CBC করার মুল উদ্দেশ্য রক্তকনিকার ঘনতা বেড়ে গেলো কিনা অথবা প্লাজমা কমে গেলো কিনা,এটা দ্রুত নির্নয় করা।
রক্তের অণুচক্রিকা শুরুতেই কমে যায় না,এটা মাথায় রাখতে হবে।
এর পরের অংশ পর্ব-২ এ।

The beauty of MEDICINE is to diagnose any case early before any complication arises,,,,

Here, is the lady of 35 years came to me with sudden gross deviation of mouth to her left side and she cant close her right eye anyway along wid she cant able to drink water as it came out from angle of right side of her mouth...

All these presentations were only for 12 hrs and she rushed to mine as her elder sister is my patient too,,,

Diagnosis is simple enough if anybody go through d case,, thats not the matter here...

Topics is, how rapid her response regarding treatment..

If she come after moving to n fro even for 5-7 days later, may be it will be difficult to correct all functions of facial groups of muscles..

Along with d diagnosis, i make confirmed that she is a case of Hypothyroidism too..

Also examined her both EAM to explore any vesicles there to read out RHS,,, nerve to stapedius is also intact as there was no hyperacusis..

Cause: mostly viral infection.

Here is her picture just after 7 days of taking medications...

She is happy enough to give permission of taking d image and advised to come 7 days later with full hearted smile,in sha ALLAH..

Message here is to diagnose any case promptly and treat early is d key to cure of a patient...
