S/P Survivor of cardiac arrest In NICU.
S/P Severe Coarctation of aorta.
Had BAP for COA in neonatal period on 2012.
Now cured.

11.08.25

3 case of PDA device closure, 2 case of BVP for PS, 1 case of ASD device closure & 1 case of VSD device closure.

1. Abdullah
- Age: 1.10 years | Gender: Male | Weight: 9.5 kg | Hb%: 10.3 g/dL
- History: Repeated RTI, Cough & runny nose for 7 days, H/O fever 7 days back.
- Diagnosis: PS -70 mmHg, PV annulus 8mm.
- Procedure: BVP for PS, Balloon size: 12x3 Tyshak-II

2. Sara Islam
- Age: 2.8 years | Gender: Female | Weight: 15.5 kg | Hb%: 13.5 g/dL
- History: Repeated RTI, H/O Febrile seizure (3 times).
- Diagnosis: Large ASD (8-10 mm), I- 14, S-11, IAS- 36mm.
- Procedure: ASD device closure, device size: 12 mm cocoon

3. Montasir
- Age: 5 years | Gender: Male | Weight: 15.5 kg | Hb%: 12.8 g/dL
- History: Repeated RTI, Chest pain, palpitation, Reluctant to feed.
- Diagnosis: Small PM VSD (4 mm).
- Procedure: VSD device closure, device size: 7/5 mm MFO.

4. Jubayer
- Age: 14 years | Gender: Male | Weight: 26 kg | Hb%: 12.9 g/dL
- History: Effort intolerance, RTI.
- Diagnosis: Large PDA (5 mm), Mild AS.
- Procedure: PDA device closure, device size: 12/20 mm cocoon.

5. Arisha Ayat (Mituli)
- Age: 6 months | Gender: Female | Weight: 5 kg | Hb%: 19.6 g/dL,
- History: Bluish coloration of skin and lips during crying, Respiratory distress.
- Diagnosis: TOF, Small PDA.
- Procedure: BVP for PS, Balloon size: 8x3 Tyshak-II .

It was very difficult case performed under morphine infusion. Severe infundibular dilatation spasm was present and RVOT crossed after trial of one hour. Later balloon valvuloplasty done with 8x3 mm Tyshak II balloon.

6. Subaita Islam Nafisa (Mituli)
- Age: 6 years | Gender: Female | Weight: 14 kg | Hb%: 12.6 g/dL
- History: Repeated RTI, H/O hospitalization (4 times) due to pneumonia and last one was 7 days back.
- Diagnosis: Small PDA (3.8 mm).
- Procedure: PDA device closure, device size: 8/6 mm cocoon.

7. Lazim (Mituli)
- Age: 5 years | Gender: Male | Weight: 14 kg | Hb%: 10.2 g/dL
- History: Repeated RTI, Effort intolerance, Chest pain, H/O Epistaxis (multiple times).
- Diagnosis: Large PDA (5 mm).
- Procedure: PDA device closure, device size: 12/20 mm cocoon.

- History: Repeated RTI, H/O fever 7 days back, ?scabies.
- Diagnosis: Large ASD-8mm, I-8,S-8, IAS-26mm
- Procedure: ASD device closure, device size: 12 mm cocoon.

- Age: 2.8 years | Gender: Female | Weight: 10 kg | Hb%: 11 g/dL
- History: Repeated RTI.
- Diagnosis: Large ASD (8 mm), I-9, S-9, IAS-28mm
- Procedure: ASD device closure, device size: 14 mm cocoon

- Age: 3 years| Gender: Female | Weight: 12 kg | Hb%: 11.5 g/dL
- History: Repeated RTI, H/o febrile seizure.
- Diagnosis: Upper muscular VSD-3.3mm
- Procedure: VSD device closure, device size: 7/5 MFO

5. Ayash
- Age: 25 Days | Gender: Male | Weight: 3.8 kg | Hb%: 14.3 g/dL,
- History: Respiratory distress, Bluish discoloration of lips and fingers during crying.
- Diagnosis: DILV, PA, D-TGA, BVF, PDA
- Procedure: PDA stenting, stent size: 3.5x15

6. Anabia
- Age: 3.9 years | Gender: Female | Weight: 13 kg | Hb%: 11.1 g/dL
- History: Repeated RTI, allergy(unknown), palpitation, reluctant to feed.
- Diagnosis: Large ASD (8.5 mm), I-15, S-8.5, IAS-32mm
- Procedure: ASD device closure, device size: 14mm cocoon

7. Habiba
- Age: 6 years | Gender: Female | Weight: 17 kg | Hb%: 11.6 g/dL
- History: Recurrent fever, poor weight gain.
- Diagnosis: Large ASD (16 mm), I-12, S-11, IAS-42mm
- Procedure: ASD device closure, tried with 20 and 24 mm device. Both device failed to fit properly. One slipped and another appeared too large .

8.Zubayed nafiz
Age: 10 years | Gender: Male | Weight: 31 kg | Hb%: 13.6 g/dL
- History: Repeated RTI, Br. Asthma, Down Syndrome, ADHD
- Diagnosis: Large PDA (5mm), Moderate AS
- Procedure: PDA device closure, device size: 8/6, 10/8

ACHD population increasing alarmingly.
In every OPD, 4-6 cases ACHD patient reports for follow up.

1. 20 years old boy had cone reconstruction of Ebstein Anomaly on 2019.
2. 18 year old girl. S/P BDG on 2007. S/P extra cardiac fontan on 2008.

Both follow up after 1 year.

2. Faiyaz Osman
- Age: 3 years | Gender: Male | Weight: 9 kg | Hb%: 12.5 g/dL
- Diagnosis: S/P BAP for CoA with 6×2 Tyshak mini balloon on 19/08/2022, Re Coarctation (70 mmHg), Coarct diameter – 5mm
- Procedure: BAP for COA, balloon size: 8x3, 10x3.

3. Fahim Shahriar Shovo
- Age: 4.6 years | Gender: Male | Weight: 10 kg | Hb%: 13.3 g/dL
- Diagnosis: Large fenestrated ASD (7.5 mm), I-10, S-14, IAS-30mm, Mild PS, Mild RPA
- Procedure: ASD device closure, device size: 12.

4. Zafirah Amatul Arusa
- Age: 12 years| Gender: Female | Weight: 50 kg | Hb%: 12.2 g/dL
Note- Mother has ASD secundum
- Diagnosis: Large ASD (15 mm), I-11, S-11, ias 45mm with keloid behind right ear (under treatment) and chest
- Procedure: ASD device closure, device size: 26.

Difficult day with difficult cases but successful.

- Diagnosis: Small PDA (3-4mm).
- Procedure: PDA device closure, device size: 8/6 cocoon

2. Marjiha
- Age: 2.3 years Gender: Female | Weight: 12 kg | Hb%: 11.1 g/dL
- History: Repeated RTI, Pneumonia for 2 times.
- Diagnosis: Large tubular PDA (8 mm), severe PAH CoA, MVP
- Procedure: PDA device closure, device size: 14x12. TR venae contractae reduced .

3. Momin Ahmed
- Age: 2.6 years | Gender: Male | Weight: 14 kg | Hb%: 11.5 g/dL
- Diagnosis: Moderate sub aortic VSD-4mm
- Procedure: VSD device closure, device size: 6/4, MFO

4. Shikh Ayat
- Age: 5days | Gender: female | Weight: 2.7 kg | Hb%: 1.5 g/dL
- Diagnosis: DORV,PVA,VSD,ASD,PDA with straddling of septal defect of TV.
- Procedure: PDA stenting , stent size: 3.5× 18.

It was difficult to cross PDA and engaged in branch PA,s . Several attempts and wire change required to achieve goal.

5. Tasfia Jannat
- Age: 3.2 years| Gender: Female | Weight: 10 kg | Hb%: 11.7 g/dL
- History: Repeated RTI, H/o unconsciousness.
- Diagnosis-? Seizure disorder with Moderate upper muscular VSD-5mm
- Procedure: VSD device closure, device size: 12x10 MFO . Initially tried with 8x6 and 10x8 device and slipped away.

6. Erfan Sheikh
- Age: 10.10 years | Gender: Male | Weight: 22 kg | Hb%: 10.8 g/dL,
- Diagnosis: Moderate mid muscular VSD-7.5mm with H/O fall from height with? GERD with Scabies.
postponed for viral hepatitis

7. Sanjida Akter
- Age: 15 years | Gender: Female | Weight: 59 kg | Hb%: 12.8 g/dL
- Diagnosis: Small PDA-4mm
-Procedure: PDA device closure, device size:10/ 8 device.

Diagnosis: Mildly trabeculated LV with non compaction.
Mother had VSD closure at the age of 7 years in 1989 by Prof Nobi Alam Khan in NICVD.
