- History: Repeated RTI, Runny nose and cough for 7 days, Feeding difficulties, Reluctant to feed, H/O Bluish discoloration of skin, H/O PNA
- Diagnosis: Large PDA-4 mm
- Procedure: PDA device closure, Device size: 8/6,

3. ALISHBA
- Age: 1.6Y | Gender: Female | Weight: 7 kg | Hb%: 11.3 g/Dl (O-VE blood)
- History: Recurrent RTI, H/O admission in NICU following birth for pneumonia, Poor weight gain, H/O food poisoning 15-20 days ago, Itchy lesion in both upper and lower limbs
- Diagnosis: Large tubular PDA-5 mm severe PAH
- Procedure: PDA device closure, Device size: 12x10

4. SAMIA
- Age: 4Y | Gender: Female | Weight: 13 kg | Hb%: 12.1 g/dL (O-VE blood)
- History: .Recurrent RTI , Cough and cold for 10 days, poor weight gain
- Diagnosis: large ASD- 14 mm, I-11 ,S-11, IAS -45
- Procedure: ASD device closure, Device size: 20 mm.

5. SAFRIN
- Age: 2.3Y | Gender: Female | Weight: 10 kg | Hb%: 12.5 g/dL
- History: Repeated RTI, Mild cough for 2 weeks, Feeding difficulties, Reluctant to feed, Poor weight gain, H/O allergy
- Diagnosis: Small Sub aortic VSD-3.5mm, Mild Infundibular Stenosis
- Procedure: VSD device closure, Device size: 7/5 MFO.

6. IMTIAZ
- Age: 7.2Y | Gender: Male | Weight: 19 kg | Hb%: 12.8 g/dL
- History: Repeated RTI, Effort intolerance, H/O Pneumonia, H/O PNA
- Diagnosis- large ASD- 22 mm, I-10, S-11, IAS -46
- Procedure: ASD device closure, postponed for floppy rims

7. AMINA
- Age: 37Y | Gender: Female | Weight: 44 kg | Hb%: 12.3 g/dL
- History: Repeated RTI, , Effort intolerance, H/O mild cough and fever for 7 days, H/O Two LUCS, Known case of Hypothyroidism for 15 years.
- Diagnosis: Atrial septal aneurysm Small ASD- 8 mm, I-32, S-17, IAS -60, PFO 2 mm .
- Procedure: A challenging case of ASA with two holes . Large ASD device closure, Device size: 16 , small PFO device closure size 10 mm device.

An interesting case of aneurysmal septum with PFO and moderate ASD SECUNDUM closed with two devices

Patient is a 37 years old lady with 45 kilogram body weight and echocardiography showed. Aneurysmal septum , mobile bulging towards RA and PFO at lower end and Moderate ASD at upper end . We planned for closure of ASD only as there was no history of any stroke or vertego. IN THE CATH LAB WE REVIEWED OUR PLAN AND WE DECIDED FOR CLOSING PFO ALSO BECAUSE OF ANY CHANCE OF STROKE IN FUTURE AS there is possibility of boarding in long flight in future. IT WAS VERY CHALLENGING TO CLOSE BOTH PFO
AND ASD IN ANEURYSMAL SEPTUM
WE FIRST CLOSED PFO WITH THE 10 MM AND THEN WE CLOSE THE SECOND ONE THE ASD OF MODERATE SIZE WITH 16 MILLIMETER ASD DEVICE

। PROCEDURE WAS VERY SUCCESSFUL AT THE END AND WHOLE ANEURYSMAL SEPTUM WAS COVERED WITH BOTH DEVICE .

- History: .Recurrent RTI , Difficulties in breathing, poor weight gain
- Diagnosis: Large Upper VSD 8 mm
- Procedure: VSD Device closure Device size: 12x10.

Procedure, LV angiogram showed large upper muscular VSD but view was hazy and no clear margin seen. Initially 8x6 device placed. But it slipped out. Later 12x10 mm device placed with difficulty though arterial approach and 5 f DS and it was successful. Small residual shunt seen but it was acceptable.

Case summary: 3 cases of PDA device closure, 2 cases of ASD device closure, 1 cases of VSD device closure & 1 cases of PDA Stenting.

1. HIMU
- Age: 9Y | Gender: Female | Weight: 37 kg | Hb%: 13.9 g/dL
- History: Recurrent RTI, cough for 3 months
- Diagnosis: Small PDA-3mm.
- Procedure: Wire failed to cross PDA . Snare assisted technique used to pull wire and catheter over the wire to aorta. PDA device closure performed later using normal techniques, Device size: 8/6.

2. MAIDA MEHRISH
- Age: 9M | Gender: Female | Weight: 5.4kg | Hb%: 11
- History: Asymptomatic, Incidental Diagnosis of at 4 months of age.
- Diagnosis: Moderate PDA-5mm, Small ASD
- Procedure: PDA device closure, Device size: 8/6.

- Diagnosis- Large PDA-9mm, Large PM VSD severe PAH reverse shunt .
- Procedure: PDA device closure, Device size: 12/10.

7. SHAMPA AKHTER
- Age: 27Y | Gender: Female | Weight: 54 kg | Hb%: 11.2 g/dL
- History: Occasional Chest pain for 2 years, Effort intolerance, H/O Lap. Cholecystectomy 1 year back.
- Diagnosis: large ASD- 24-26 mm, I-14, S-16, IAS -55
- Procedure: ASD device closure, Device size: 32 mm.

7 years after Melody Valve Imaplantation.

Patient S, 17 yrs old young lady, 49 kg weight was diagnosed as a case of Pulmonary Atresia with VSD, her Intracardiac repair with RV to PA conduit was done in 2014, later in 2019 her PPVI with Melody Valve, then Balloon dilatation of Melody valve done in year 2021. Now she came for follow up and her recent echo shows Mild Melody Valve stenosis, gradient 19 mmHg with Mild to moderate AR.

Summary. Two cases of PDA device closure, 3 cases of ASD device closure 3 cases of VSD device closure and one case of critical PDA Stenting.

1. Nafisa
- Age: 7m | Gender: Female | Weight: 4 kg | Hb%: 11.7 g/dL
- History: Recurrent RTI, cough for 7 days ,vomiting
- Diagnosis: large PDA-3mm.
- Procedure: PDA device closure, Device size: 8/6,MFO

2.Shahadat
- Age: 2.6 Y | Gender:male | Weight: 8.5 kg | Hb%:
- History: , Recurrent RTI respiratory distress, FTT, oral ulcer .
- Diagnosis: mod PM VSD
- Procedure: VSD device closure, Device size: 7x5 . It was very difficult to cross VSD . Tried with different Catheter and different techniques. Lastly possible to cross just before postponing the case .

3.WAZIHA
- Age: 3M | Gender: Female | Weight: 4.1 kg | Hb%:
- History: Respiratory distress, H/o PICU admission due to Pneumonia, Recurrent RTI, & S/P cyanotic spell
- Diagnosis: PA,DORV,VSD,PDA,EA
- Procedure: PDA STENTING 4/18

PDA was tortuous with 3 curves . It was difficult to cross PDA and it took 2 hours to cross ultimately. Double wire technique used . PDA forwarded over the wire and placed inside PDA .

4.Afshia
- Age: 2.4Y | Gender: Female | Weight: 8.4 kg | Hb%: 11.1 g/dL
- History.Recurrent RTI ,poor weight gain
- DiagnosisLarge ASD 10 mm,I-12.5 S-8.5 IAS 33

- History, Repeated RTI, AGE for 3 days 2 days ago, H/o pneumonia once.
- Diagnosis: Moderate PDA (3.5 mm)
