Patients were positioned laterally, supine, or even seated in straddling chairs, while radiation was delivered through large external cones. Although innovative for their time, these approaches lacked the precision and safety standards of today.

Modern treatment has transformed dramatically. Intensity-modulated radiotherapy (IMRT), stereotactic body radiotherapy (SBRT), and proton therapy now deliver highly targeted doses, minimising damage to surrounding healthy tissue while improving disease control. Advances in surgery—such as laparoscopic and robotic-assisted techniques—further reflect a shift towards safer, less invasive, and more patient-centred care.

This striking contrast between past and present serves as both a reminder of medicine’s progress and an inspiration for continued innovation in the fight against colore**al and a**l cancers.
