“Customised Subperiosteal Implant for Posterior Mandibular Rehabilitation Following Repeated Graft Failure.
Patient Profile
A 66-year-old male patient presented with the chief complaint of inability to chew efficiently and a strong desire for a permanent prosthetic solution.
Clinical History
The patient had previously undergone three bone grafting procedures in the mandibular posterior region, all of which resulted in failure.
Clinical and radiographic evaluation revealed:
Residual bone height of 6.6 mm and 5.4 mm in the posterior mandible
Inadequate bone volume for conventional endosseous implant placement
Presence of a periodontally compromised tooth (47).
Treatment PlanningConsidering:
Repeated graft failure
Severely reduced vertical bone height
Patient’s demand for a fixed, long-term solution
A customized subperiosteal implant was planned for the mandibular posterior segment, along with extraction of the compromised tooth. endoosteal implant was planned for other missing areas.
Surgical procedure:Tooth 47 was extracted prior to implant placement.
A custom-designed subperiosteal implant framework was fabricated based on the patient’s anatomy.The implant was positioned over the mandibular bone and stabilized using 8 mm fixation screws. Proper adaptation and primary stability were achieved. A titanium bar (framework) was connected to the implant abutments .Final prosthesis was delivered using a screw-retained mechanism.
Achieved:
Good occlusal stabilityFunctional rehabilitation
Patient satisfaction.

A part of jaw has been replaced with patient specific subperiosteal implant.

🚨 মুখের ক্যান্সার সচেতনতা 🚨
❗ প্রাথমিক লক্ষণগুলো কখনোই অবহেলা করবেন না
মুখের যেকোনো সমস্যা যদি ২ সপ্তাহের বেশি স্থায়ী হয়, তাহলে দেরি না করে অবশ্যই বিশেষজ্ঞ ডাক্তারের পরামর্শ নিন।
🔍 যে লক্ষণগুলো দেখলে সতর্ক হবেন:
• মুখের ঘা যা ২ সপ্তাহেও সারে না
• মুখের ভেতরে লাল বা সাদা দাগ
• গলায় ফোলা বা গাঁট
• গিলতে সমস্যা হওয়া
• কণ্ঠস্বরে পরিবর্তন বা ভাঙা ভাঙা আওয়াজ
• মুখ ঠিকমতো খুলতে না পারা
• হঠাৎ দাঁত নড়বড়ে হয়ে যাওয়া
💡 মনে রাখবেন:
প্রাথমিক অবস্থায় রোগ শনাক্ত করা গেলে জীবন, কথা বলা, খাওয়া এবং চোয়ালের স্বাভাবিক কার্যক্ষমতা অনেকাংশে রক্ষা করা সম্ভব।
⏰ তাই দেরি না করে—
২ সপ্তাহের বেশি কোনো সমস্যা থাকলে অবশ্যই ডাক্তারের শরণাপন্ন হোন।
