A case of 40 days old comminuted (an additional coronal plane fracture)lateral condyle fracture of humerus of a 5 years old boy.
The fragments were very much unstable.
Patient couldn’t extend his elbow.
So, to stabilize the fracture fragments additional 2 k wire had to be fixed.
Patient's guardian has been counselled regarding any unwanted & unpleasant outcome.
Alhamdulillah, Pt now can extend the elbow.
Rest of the result is upon Allah Subhana Ta-ala regarding everything, specially union, growth retardation, deformity.

This was a case of 18 days old open infected comminuted fracture of distal femur(G-ii).with DM.
ESR -130mm, CRP-68.
Patient refused for ILIZAROV Ring Fixator
So, Extensive debridement was done & External fixator applied.
After 1 month, ESR declined to 35mm & CRP became negative.
Then Open reduction & internal fixation done with Distal Femoral Locking plate & screw with massive autogenous cortico-cancellous bone graft.
Alhamdulillah, no signs of infection noted till last 7 days.

A case of 1 year old Inter trochanteric fracture with 7 cm shortening of a 50 years old lady.

But, dramatically patient could walk with support and straight leg rising was possible that may misguide any orthopaedic surgeon to be a case of malunion. Patient eagerly seeking for limb length equality and walk without support.
1.The patient could walk as the proximal femur acts as a strut with the iliac plate.
2.During the leg rising, a palpable crepetus could be noticed at the fracture site. With firm pressure, I found that the proximal fragment doesn’t move with the whole limb.
Those 2 issues lead me to think this case differently.
I did an xray with 7.5kg of traction. &
The proximally migrated distal fragment came down with the load. I finally diagnosed the case as nonunion (though rare).
The patient was advised for home traction with 5 kg for 3 days then gradually waight was increased.
I was worried about the cartilagenous congruity of the acetabulum. As, the fracture is extra capsular, the acetabulum should be intact, (I believed).
Then I went for surgery.
Firstly, I was so happy to find out that the cartilage of acetabulum was intact.
2nd challenge was to reconstruct the rounded & sclerosed (due to repeated stuck with iliac plate during weight bearing) proximal femur with bone cement.
Finally hemiarthroplasty was done with cemented prosthesis. Traction was maintained for 3 weeks post operatively.
Finally, the patient walked with same leg length, Alhamdulillah.
All that meant to me was the smile of satisfaction of the patient.
