★ ৪ঠা ফেব্রুয়ারী বিশ্ব ক্যান্সার দিবস।
★ ২০২৫ থেকে ২০২৭ সাল পর্যন্ত এই দিবসের প্রতিপাদ্য নির্ধারন করা হয়েছে "United by Unique " অর্থাৎ "স্বকীয়তায় ঐক্যবদ্ধ"।
★ মানবদেহের সর্বমোট ক্যান্সার গুলোর মধ্যে এশিয়ায় ২৫ শতাংশ এবং ইউরোপে ৬-৮ শতাংশ রোগী হেড-নেক (নাক-কান-গলা) ক্যান্সারে আক্রান্ত।
★ মনে রাখবেন প্রাথমিক অবস্থায় নাক-কান-গলা ক্যান্সারের যথার্থ চিকিৎসা করলে পুরোপুরি এই ক্যান্সার থেকে মুক্ত হওয়া যায়।
★ বর্তমানে বাংলাদেশে সরকারী ও বেসরকারী পর্যায়ে হেড-নেক (নাক-কান-গলা) ক্যান্সারের সর্বাধুনিক চিকিৎসা ব্যবস্থা বিদ্যমান।

গলার ডান দিকে ফুলে গেছে প্রায় ৬ মাস।

#থাইরয়েড_আলট্রাসোনোগ্রাম :
থাইরয়েডের ডান লোবে অসংখ্য শক্ত নডিউল রয়েছে। যা প্রায় ৬.৪x ৪.৩ মি.মি, ৮.২ x ৮.২ মি.মি

Follicular variant of Papillary thyroid carcinoma.

45 years old, A female patient came with the complain of swelling in right side of the neck for 6 months.

:
★Both the lobes of thyroid gland appear normal in size (RL-32 x 11 mm, LL-29 x 11 mm).
★Multiple solid nodules of variable size are noted in right lobe, solid component measuring about (6.4 x 4.3 mm, 8.2 x 8.2 mm etc.) noted in right lobe.
★Isthmus appears normal (3.5 mm).
★No cervical lymph-adenopathy could be noted.
★Major neck vessels appear unremarkable.

From Thyroid Swelling :
Bethesda Category-V, Suspicious for Follicular variant of Papillary thyroid carcinoma.

থাইরোগ্লোসাল সিস্ট হলে আমাদের গলার ঠিক মাঝ বরাবর ছোট্ট একটি অংশ ফুলে যায়। এই ফুলে থাকা অংশটিই হলো সিস্ট। এর ভেতরে পানির মতো তরল পদার্থ থাকে। সাধারণত শিশুর জন্মের পর এই সমস্যা দেখা দেয়। তবে পরিণত বয়সেও এই সমস্যা দেখা দিতে পারে। এটি আসলে একটি জন্মগত ত্রুটি। থাইরয়েড হরমোন গ্রন্থিটির নিচে নামার পথটি রয়ে গেলেই এই সমস্যা দেখা দেয়। শিশু ঢোক নেওয়ার সময় অথবা জিহ্বা বের করলে এই সিস্টটি গলার সামান্য ওপরের দিকে উঠে আসে।

এটি দেখেই চিকিৎসকরা রোগটি শনাক্ত করে থাকেন। অস্ত্রোপচার করে সিস্টটি ফেলে দেওয়াই এটির একমাত্র চিকিৎসা। এই অস্ত্রোপচারের নাম সিস্ট্রাঙ্ক অস্ত্রোপচার।

A Female, 46 years old, Came with the complain of swelling infront of the neck for 3 months.

USG of Neck: Left lobe: Single solid heterogenous nodule with calcification is seen in lower pole of left lobe measuring about (1.6×1.2cm), also margin is ill-defined.

USG guided FNAC from left lobe of thyroid: Follicular neoplasm. Differential diagnoses include: Follicular variant of papillary carcinoma and follicular adenoma.

Our plan of Care-
Left Hemithyroidectomy under G/A (According to ATA guideline, low risk tumor, low risk patient)

A Female patient, 47 years old came with the complaints of Swelling infront of the neek 5 months.

Previous Operation History: Rt Hemithyroidectomy on 2015

Of_Thyroid_Gland:
Left lobe of thyroid gland is enlarged in size having hypoechoic (15 x 15 mm) & complex. Cystic nodular lesion (16 x 12 mm) with septations. Color flow shows peripheral vascularity. Rest of the thyroid shows inhomogeneous echo pattern.
Left lobe measuring about - 3.4 x 2.0 x 2.0 cm. Volume is about - 7.2 cc.
Neck lymphnodes: No evidence of any enlarged neck nodes were visualized on either side of the neck.

- Two hypoechoic well defined and oval mass lesions measuring about (1.6cm×0.7cm) and (0.5cm) Left lobe - Two hypoechoic well defined rounded mass lesions measuring about (0.4cm)×0.3cm) and (0.3cm×0.3cm) (24.08.23)

-Right lobe-Follicular lesion of undetermined significance (Bethesda classification Thy-3) [09.09.2023]

: Largest lobe of left size measuring about (47x29mm), Single calcification is seen in nodule in left lobe.
Diffuse multinodular goiter [30.09.2023]

-Thyroid swelling-right lobe: Consistent with papillary carcinoma thyroid [14.10.2023]

: Both lobes of thyroid gland are enlarged in size, more on left (9.1×5.3)cm including isthmus. Large lobulated soft tissue solid mass having focl of calcification noted within thyroid gland causing compression and displacement of trachea towards right, effacement of left carotid space, aryepiglottic fold and regional muscles which extend into superior mediastinum. After IV contrast heterogeneous enhancement of the lesion seen. [29.10.2023]
