Shotgun pellet was removed successfully from the left temporal region of a student's head. It was quite impalpable and incision was made with assumption with student's identified location. Usually the pellets are located in the subcutenous region but in this case it was embeded within temporal bone! It is usually not needed to remove the pellet until casuing problems. The patient was eagerly desire to remove this.However it was removed and he was very happy and free from anxiety.

On day before yesterday I was performing an open cholecystectomy. Every thing was fine. All the steps were unevenful and the gall bladder was removed. Its time to close. But just before closing, my curious mind have seen something unusual! Look further check up and found leakage of bile close to gb fossa away from the duct. The leakage duct was too tiny to miss easily. Initally thought to be liver/duct injury but it was actually the Aberrant billiary channel! The duct was closed with suture and a drain was kept in situ before closing the wound. Drain collection was about 40 cc(serous fluid. Not bile)on first POD and nil on 2nd day. The patient was then discharged after removing drain tube.
Its a quite bit experience gained! Carefull check up is a must. It might have missed!

Hernia
পেটের ভেতরের কিছু অংশ যখন বাহিরে (চামড়ার নিচে )বের হয়ে আসে তখন তাকে আমরা hernia বলে থাকি। ছোট থেকে বড় যেকোনো বয়সেই এটা হয়ে থাকে।
কিভাবে বুজবেন? দেখবেন শুয়ে থাকলে দেখা যায় না কিন্তু হাঁটা চলা করলে /হাঁচি কাঁশি দিলে দেখা যায়।
Hernia বিভিন্ন ধরণের হতে পারে তবে কুচকির (inguinal ) হার্নিয়া সবচেয়ে বেশী পাওয়া যায়। তাছাড়া আগে কোনো অপারেশন হয়ে থাকলে সেই অপারেশন সাইট এও হার্নিয়া হতে পারে। নাভি এবং নাভির আসে পাশে কিংবা পেটের উপরিভাগেও হার্নিয়া হতে পারে!

An interesting case scenario:

The day before yesterday, a female patient of 40 years came to my chamber with the complaints of abdominal fullness, discomfort with occasional pain for last few months. On examination she was anaemic with palpable large abdominal lump about 20*15 cm occupying umbilical, suprapubic and rt and lt lumber region! It was firm in consistancy and moved from side to side and very little to above downwards.
I advised few test along with ultrasound. After seeing the report i was shocked! It was single live pregnancy of 30+ weeks!!! She never gave any history regarding pregnancy and she was unware about it!! On further query she mentioned her cycle was regular all that time period with average flow rate. She was married for 18 years with having no child. Then further advised serum beta hcg and it was strongly positive according to fetal age.

My patient Mr. X ,55 yrs was suffering from this lesion for last 20 yrs in his forehead. This gradually increases in size and appear upto 5×4 cm. The lesion was painless.On examination it was farm in consistency,very slightly mobile and bony indentation. There was no punctum. My diagnosis was epidermal inclusion cyst. A young boy of 12 years age came to me with partial amputation of left index finger during chopping of something. Distal phalanx fragments are stabilized by K-wire fixation. An excellent allaignment is obtained and the wound is closed.
