31-years-old woman has presented with chronic diarrhoea & abdominal pain for 18 months. Stool is semisolid, moderate volume, non-bloody, varies 2-8 motions/day. Associated cramping abdominal pain causes sleep disturbance. Despite this she has no weight loss & no H/O fever. [She has some arthralgia without any objective abnormalities of joint or no other features to suggest SLE] Examination reveals Anemia ++, & mild generalised abdominal tenderness. CBC: Hb-7.2 (then received 2 unit blood). ESR-raised, ranges 50-90. LFTs, RFT, RBS normal. Stool OBT -negative (once)
Received metronidazole, Azithromycin, ciprofloxacin without significant improvement. Never received steroid. Colonoscopy picture is shown with a biopsy report of non-specific ulcers.

What are the possibilities?
Plan of treatment / further investigations?
