📣 EAHP published a new position paper on Patient Safety
EAHP highlights several key recommendations to improve safety, reduce medication errors, and ensure high-quality care in hospitals:
🧑⚕️ Guarantee an adequate number of hospital pharmacists and address workforce shortages to improve patient care
🏥 Involve hospital pharmacists in clinical risk working groups to improve personalised and patient-centred care
⚠️ Apply different risk management tools
📝 Avoid transcription and duplication of prescribed medicines
💊 Uptake closed loop medication management for a faster and more accurate administration of medicines.
🔬 Implement clinical pharmacy services to ensure medication appropriateness, reconciliation and personalisation.
👫 Strengthen patient engagement for increasing patient-safety through a patient-centred approach.
🩺 Strengthen interprofessional collaboration
🦠 Reduce Healthcare-Associated infections (HAI) through infection prevention and control measures.
🔐 Make use of innovation while guaranteeing patient safety in all technological applications through the enforcement of data privacy and cybersecurity and the organisation of training for healthcare professionals and patients on the safe and correct use of new technologies.
