How prevalent is intra-abdominal hypertension (IAH), and at what point does it progress to abdominal compartment syndrome (ACS)?

These questions remain challenging, given varying criteria and a “gray zone” between mild elevations in intra-abdominal pressure (IAP) and severe organ dysfunction.

🔹 Variability in Definitions: Different cutoff values (12, 15, 20, or even 25 mmHg) complicate comparisons of IAH incidence, yet studies consistently show a higher prevalence following emergency surgery.

🔹 Progression to ACS: IAH becomes ACS when increased IAP coexists with organ dysfunction. While decompression is advised for IAP >20 mmHg with clear organ compromise, patients in the 15–20 mmHg range often require clinical judgment due to borderline symptoms and multiple confounding factors.

🔹 Clinical Implications: The “no-man’s land” between normal IAP and ACS highlights the importance of more precise measures (e.g., splanchnic perfusion) to guide timely interventions and optimize patient outcomes.

Our protocol details a systematic approach to intra-abdominal pressure measurement using indirect intravesicular methods. Essential equipment includes a cardiac monitor with invasive pressure recording, pressurized IV solution, a Foley catheter, and a dedicated transducer setup. The procedure follows stringent aseptic techniques and careful patient preparation. Key steps involve zeroing the pressure transducer at the symphysis p***s, controlled instillation of 50 mL IV fluid into the bladder, and verifying the waveform through gentle abdominal oscillations. The mean IAP is recorded at end-expiration and interpreted against established clinical benchmarks to aid in the early detection of intra-abdominal hypertension.
