Most patients don’t realise how much temple points influence what they’re seeing.

When they’re right, everything looks balanced.
When they’re wrong, it can be a disaster.

And this is where a lot of decisions go wrong.

Temple points are not for everybody.

They can require a significant number of grafts on each side, and those grafts come from a limited donor. So the question isn’t just ,will they look good?
It’s, are they right for you, given your age, your pattern of loss, and what you may need in the future?

In some cases, rebuilding them frames the face beautifully.
In others, a more conservative approach is the smarter, more sustainable decision.

Technically, their position isn’t guesswork.

There’s a natural reference: a line drawn from the tip of the nose, through the centre of the eye, and beyond.
That helps guide where a temple point should sit to look natural within the face.

In this case, everything was planned and aligned - hairline, temple points, direction, angulation — before a single graft was placed.

This patient (mid-30s) presented with temporal recession and loss of balance through the frontal hairline.

A total of 2,519 FUE grafts were used to restore the temples and reinforce the frontal zone.

Planning and recipient site creation were performed by Dr. Bisanga, with graft extraction and placement carried out alongside Dr. Abby.

The objective was not simply to add density, but to recreate a hairline that reflects the patient’s original pattern, existing hair, and long-term progression.

What makes this result believable is not the graft count.

It’s how the work was designed and executed:

– the hairline follows his original shape, rather than a generic template
– direction and angle are matched to how his hair naturally grows
– density is planned

Most people think a consultation is where you get a number.

“How many grafts do I need?”

That’s it.

No real measurement.
No breakdown.
No understanding of what’s actually possible.



A real consultation looks very different.

It looks at:
* Your donor density (what you actually have to work with)
* Your hair groupings (how much hair each graft gives)
* Miniaturisation (whether your donor is even stable)
* Scalp health (factors that affect growth and yield)
* Your long-term loss pattern (not just today)

Only then does a graft number mean anything.



The problem is, most patients don’t know this.

So they compare clinics based on numbers alone…
and assume they’re being assessed properly.

They’re not.



If a clinic gives you a number without measuring these factors first, it isn’t a plan.

It’s a guess.

This patient came to us with one clear objective:

This is where most people get it wrong.

They focus on the number.

But graft numbers alone don’t determine your result.

Hair restoration is about creating an illusion of density, not restoring your original density. Your donor simply wouldn’t support that.

So instead, the result comes down to how those grafts are used.

Surface area plays a huge role. The larger the area that needs to be covered, the more those grafts have to be spread out… which means less density per cm².

That’s why two patients with the same graft count can end up with very different visual outcomes.

But it doesn’t stop there.

Hair calibre matters. Thicker hairs create more visual coverage than finer hairs.

Hair groupings matter. A graft containing multiple hairs will contribute more density than a single-hair graft.

Hair colour and skin contrast matter. Dark hair on light skin shows thinning much faster than lighter combinations.

And placement strategy matters. The hairline, transitional zone, and mid-scalp are all treated differently to create a natural result.

So when someone asks:
“Is 3000 grafts enough?”

The honest answer is…

It depends.

Not on the number.

Why the beard can change everything 🧔‍♂️

Does it leave scars?

When graft demands are high or the scalp donor supply is limited, the beard can become a valuable additional donor source.

Beard hair shares many similarities with scalp hair. It follows the same growth cycle (anagen, catagen and telogen), which means once transplanted into the scalp it can continue to grow in a stable and predictable way.

Another advantage is thickness. Beard hair is typically coarser than scalp hair, which can contribute significantly to the visual appearance of density , particularly in areas like the mid-scalp or crown where supporting coverage is needed.

However, beard hair has its own characteristics. Curl, coarseness and colour must always be considered when planning surgery. Surgeons carefully place beard grafts in areas where these characteristics blend naturally with surrounding hair.

Beard grafts are usually used as a supplementary donor source, not a replacement for scalp donor hair. By combining both strategically, surgeons can expand the total graft supply and address larger areas of hair loss.

This can be particularly valuable in repair cases, where the scalp donor area may already be depleted.

And what about scars?

Extraction sites are extremely small and spaced throughout the beard area. Once healed, they are typically very difficult to detect even with shorter facial hair, as seen in this patient.

When used correctly, beard hair can significantly expand donor supply and allow surgeons to achieve natural and balanced results.

This patient first visited the clinic presenting with a Norwood 6 pattern of hair loss and a relatively limited scalp donor area, with thinning behind the ears and dipping into the lower crown. Cases such as this require careful long-term planning and a very conservative approach to donor management.

Dr. Bisanga approached this restoration across three separate sessions, strategically managing the scalp donor while also incorporating the all-important beard donor resource. In total, 6,799 grafts were harvested from the scalp and a further 2,204 grafts from the beard to build coverage and density across a very large surface area.

Extracting this number of grafts while preserving the integrity and appearance of the donor area is one of the greatest technical challenges in hair restoration. Donor management, graft selection, and long-term planning are critical in cases like this to ensure that the scalp continues to look natural and healthy after surgery.

For the patient, the impact of a transformation like this can be profound. Moving from an advanced pattern of loss to restored framing of the face can dramatically change how a person feels about their appearance and their confidence day to day. 2 years after surgery and the result speaks for itself 👌

Performed by Dr Kostis, this case involved 2500 grafts to restore and refine the hairline while maintaining a natural, age appropriate design.

Dr Kostis focused on creating a hairline that would look natural not only today but also for the future. Careful attention was given to graft placement, density distribution and very importantly the direction and angulation of the hair to follow the patient’s natural growth patterns. Complex elements such as cowlicks in the frontal hairline were respected to ensure the result blends seamlessly with the patient’s native hair. 🎯

This patient had experienced recession from a young age and had been stabilising his hair loss with finasteride since 2016, later adding minoxidil following surgery. At age 35 he decided to move forward with restoration after years of research and consultation.
