"🔎Relatively recent research demonstrating a mechanism through which exercise may work to reduce the risk of osteoarthritis. This study fits nicely with others showing that moderate exercise appears to have a protective effect on joint cartilage.

🧠In this study (cited below), researchers subjected chondrocytes (cartilage cells) to mechanical loading, which activated the HDAC6 protein and lead to a suppression of inflammatory signaling.

🏋🏼‍♂️Often times, I run into people who worry that exercise will damage their joints. Hopefully, this research will change this belief and help others understand that loading their joints through moderate doses of exercise can positively influence cartilage health and reduce the risk of developing arthritis.

Can this damage your discs?

No.. 😊
A lot of physiotherapists, doctors, orthopaedic surgeons, and the general public have to change their beliefs about the spine.

People with degenerative discs have been told that they can’t bend their back, lift heavy and do manual work.

There are a lot of examples of people who have gone back to doing the very things they’re being told they can’t do.
There is a lot of fear about the structure of the spine.
They have been told to be careful with their backs and have the fear to do harm.

We know that fear is a huge driver of abnormal pain behaviour.

A shift in beliefs has to happen within the medical, physiotherapy profession and in the general public.

We can trust our back and shouldn't be feared. It is a robust, strong structure.
Often with disabling back pain, the motor systems are overactive, hypersensitive. The muscles are guarding the back too much.
That can be driven by habits of movements, habits of posture, fear, anxiety, stress, or just the presence of pain and you developed a protective guarding response.

In this video prof. Dr. Peter O’Sullivan explains more about this problem and tells about his Cognitive Functional Therapy approach, which shows great results for patients with chronic low back pain:

📌 In 2014, a study by Darlow et al was published in BMJ Open. The study looked at beliefs of back pain in New-Zealand citizens above 18 years of age

📊 602 valid responses were received. Although not all of them had back pain at the moment (27%, which is still a substantial amount), 87% had experienced it at least once in their lives

⁉️ 34 questions were asked. A few of the most shocking results can be seen in the picture

📊 Negative views about the back and back pain were prevalent, in particular the need to protect the back to prevent injury

😩 People with current back pain had more negative overall scores, particularly related to back pain prognosis (outcome)

"IT DEPENDS…

In the fitness industry, we may find ourselves frequently wanting to classify certain movements into a dichotomy: “good” or “bad.” The issue with this thought process is ignoring the human-being behind the movements. Each individual will have a unique training and medical history, and different goals, making their exercise and fitness selection different than someone else’s. While it may not be appropriate for one person, it can actually be quite valuable for another.

Two common gym movements that seem to gain more traction as “good” or “bad” are loaded lumbar flexion (low back rounding) and knees over the toes. While the information surrounding these topics exceed that of a social media post, here are some considerations to think about:

1️⃣ Loaded lumbar flexion. Generally, if the load is light (relative to person) and someone is not flexion sensitive, there should be no concern with this movement. Furthermore, when implemented correctly, it can be a powerful exercise selection for those rehabbing from low back injuries, especially with those demonstrating fear-avoidance behavior with spine flexion. An example for everyday life would be picking up an object from the floor. Is it necessary to hinge or squat or would bending over to pick it up be okay? It depends on the person…

2️⃣ Knees over toes. A lot of activities we perform day-to-day require our knee to travel over the toes (i.e. stairs), as well as various sports (i.e. basketball, weightlifting). For training goals, knee over the toes actually allows us to dominate movements like squats and lunges with the knee, further utilizing our quadricep muscles (front of thigh). For rehab goals, progressing to knees over toes is a common treatment strategy for patella tendinopathy. Will some people be more sensitive to these movements? Absolutely. But in some cases, allowing the knees to move over the toes is ideal. Again, it depends…

Remember the human body is strong, adaptable, and resilient. To classify certain movements as “good” or “bad” is not taking the whole picture into consideration. Next time you see someone performing an exercise you deem “bad”.”
