Mortise X-ray view
A mortise X-ray view is a specialized ankle projection, typically an AP (anterior-posterior) view taken with 15–20° of internal rotation, designed to visualize the joint space between the talus and the malleoli clearly.

✅Patient position
✔️the patient may be supine or sitting upright with the leg straightened on the table

✔️the leg must be rotated internally 15° to 20°, thus aligning the intermalleolar line parallel to the detector. This usually results in the 5th toe being directly in line with the centre of the calcaneum

✔️internal rotation must be from the hip; isolated rotation of the ankle will result in a non-diagnostic image

✔️foot should be in slight dorsiflexion

Codman & Neer classification

The Neer classification system, based on Ernest Codman’s earlier anatomical work, categorizes proximal humerus fractures based on the displacement of four major segments: the greater tuberosity, lesser tuberosity, articular surface, and humeral shaft. A segment is considered "displaced" if it is separated by >1 cm or angulated > 45∘> .

✅Codman classification
✔️Articular Surface (Humeral Head)
✔️Greater Tuberosity
✔️Lesser Tuberosity
✔️Humeral Shaft

✅Neer Classification
✔️1-Part (Minimally Displaced): Any number of fractures, but no segment is displaced > 1 cm or angulated > 45∘ .
✔️2-Part: One segment is displaced in relation to the others (e.g., anatomical neck, surgical neck, greater tuberosity, or lesser tuberosity).
✔️3-Part: Two segments are displaced in addition to the shaft; often the lesser tuberosity and shaft are attached, while the greater tuberosity and articular surface are displaced.
✔️4-Part: All four segments are displaced. This carries a high risk of avascular necrosis (AVN).
✔️Fracture-Dislocations: Classified separately as 2, 3, or 4-part, including anterior/posterior dislocation or articular surface splitting.

Maisonneuve fracture refers to a combination of a fracture of the proximal fibula together with an unstable ankle injury , often comprising ligamentous injury (distal tibiofibular syndesmosis, deltoid ligament) and/or fracture of the medial malleolus.
It is caused by a pronation-external rotation mechanism.

Deltoid Tuberosity Index
The Deltoid Tuberosity Index (DTI) is a reliable radiographic tool used to assess local bone mineral density (BMD) in the proximal humerus, specifically to predict stability in proximal humerus fractures and risk of screw cutout (failure). A DTI value lower than 1.4 indicates low local bone quality, suggesting increased risk for iatrogenic fractures or fixation failure.

✅Definition: It is calculated by dividing the outer cortical diameter by the inner endosteal diameter at the widest point of the deltoid tuberosity. 

✅Measurement: It is generally taken from AP radiographs of the shoulder in internal rotation. The formula is expressed as:
DTI = a/b
✔️a (Outer Diameter): The total width of the humeral shaft from the outer edges of the cortex.
✔️b (Inner Diameter): The width of the inner medullary canal (endosteal diameter) at the same level

✅The Frykman classification categorizes distal radial fractures into eight types (I-VIII) based on intra-articular involvement (radiocarpal/DRUJ) and the presence of an ulnar styloid fracture. It progresses from extra-articular (Type I/II) to intra-articular (III-VIII), with even-numbered types including a ulnar styloid fracture.
