Case Record of the Massachusetts General Hospital: A 52-year-old man presented to the emergency department of Centro de Educación Médica e Investigaciones Clínicas (CEMIC) in Buenos Aires in early autumn with fever that had persisted for 1 week.

The patient had been in his usual state of good health until 7 days before the current presentation, when fever developed. He presented to the emergency department of CEMIC for evaluation. The temperature was 38.0°C, and testing of a nasopharyngeal swab for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RNA was negative. Supportive care was recommended.

Did you take part in this week's radiology image challenge?

The answers are:

1. a. Mediastinal and lung parenchymal abnormality.

The main abnormality is the extensive right paratracheal and bilateral hilar lymphadenopathy. In addition, there is fine nodular shadowing throughout both lungs and there is a one centimetre ill-defined opacity in the right mid zone.

2. d. Sarcoidosis.

Possibilities to consider in a patient of this age include sarcoidosis and tuberculosis, of which sarcoidosis is the most likely given the symmetrical nature of the lymphadenopathy (Garland’s triad).

This patient was diagnosed with sarcoidosis. Note on the selected HRCT image from the same patient the diffuse nodularity (red arrow), the interlobular septal thickening (blue arrow) and fissural nodularity (yellow arrow).
