🇧🇷 A projeção ântero-posterior do perfil facial é algo que eu tinha muita dificuldade de definir quando era residente… quanto avançar a maxila?

Um paciente com overjet negativo importante, podemos avançar 3mm ou 4mm ou 5mm… qual vai ser o melhor resultado?

O Arnett estipula que você use uma Linha que passa pelo subnasal do paciente, famosa Linha Vertical Verdadeira (LVV). Entretanto em casos de grande deficiência você deve mudar 3mm o posicionamento da linha… (???)

Como podemos garantir a precisão do planejamento se utilizamos uma referência que pode ser alterada a depender do problema?
Por isso é importante termos o conhecimento de diferentes análises faciais!

🇺🇸 The anteroposterior projection of the facial profile is something that I had a lot of difficulty defining when I was a resident… how much to advance the maxilla?

In a patient with a significant negative overjet, we can advance 3mm or 4mm or 5mm… what will be the best result?

Arnett stipulates that you should use a Line that passes through the patient’s subnasal area, the famous True Vertical Line (TVL). However, in cases of major deficiency you must change the positioning of the line by 3mm… (???)

How can we guarantee planning accuracy if we use a reference that can be changed depending on the problem?
That’s why it’s important to have knowledge of different facial analyses!

🇧🇷 Assimetrias Faciais sempre são um desafio quando falamos de cirurgia ortognática. Mesmo com o planejamento virtual, atingir a excelência no resultado é muito difícil.

Nessa sequência coloco um artigo que fala sobre o posicionamento da cabeça em casos de assimetria facial e como a posição natural pode mudar do pré para o pós. Na metodologia foram 31 pacientes assimétricos e 27 pacientes simétricos. E resumindo, o grupo de pacientes assimétricos tendiam a ter uma posição natural menos desviada no pós-operatório. (Artigo vai ficar no canal de transmissão - OrtogON)

E se arrastarem pro lado coloquei um exemplo da foto natural X a foto ajustada. E no slide seguinte o pré e pós sem ajuste algum da foto. Sensacional a correção da “pseudo-distopia” pelo equilíbrio mais próximo da simetria facial.

🇺🇸 Facial asymmetries are always a challenge when we talk about orthognathic surgery. Even with virtual planning, achieving excellent results is very difficult.

In this sequence I include an article that talks about the positioning of the head in cases of facial asymmetry and how the natural position can change from pre to post. In the methodology there were 31 asymmetric patients and 27 symmetric patients. In short, the group of asymmetric patients tended to have a less deviated natural position postoperatively. And if you drag it to the side, I’ve put an example of the natural photo vs. the adjusted photo. And on the next slide, the pre and post without any adjustments to the photo. The correction of “pseudo-dystopia” through a balance closer to facial symmetry is sensational.

Uma etapa fundamental da nossa analise facial e planejamento é o que chamamos de análise facial numérica. Na vista frontal fazemos um sombreamento das fotos em repouso e sorrindo para quantificar os números necessários para nos aproximarmos da simetria facial.

A fundamental step in our facial analysis and planning is what we call numerical facial analysis. In the frontal view we shade the photos at rest and smiling to quantify the numbers needed to get closer to facial symmetry.

🇧🇷 Que caso meus amigos! E super merecido 💪🏻💪🏻💪🏻 gente boa demais demais .stefanello , e põe merecido nisso 😂

Alguns vão lembrar, no ano passado quando estávamos planejando ainda, foi um dos casos que usamos no nosso curso de Análise Facial. E olha só que resultado!

A classe III esquelética quando acontece por prognatismo, as vezes encontramos excessos mandibulares tanto na posição anterior quanto na vertical. Isso muitas vezes traz esse aspecto em C para a face com a falta de projeção na região do terço médio, e o excesso de projeção no terço inferior.

Some will remember, last year when we were still planning, it was one of the cases we used in our Facial Analysis course. And look at the results!

When skeletal class III occurs due to prognathism, we sometimes find mandibular excesses both in the anterior and vertical positions. This often brings this C-shaped appearance to the face with a lack of projection in the middle third region, and excess projection in the lower third.

🇧🇷 Vista frontal: Análise Facial Numérica.

Aqui é algo matemático. Depois de posicionarmos a cabeça para iniciar o planejamento de maneira adequada, devemos nos atentar aos desvios e assimetrias que podem estar presentes na face.

Aproximar as estruturas da linha média facial e tentar buscar a simetria entre as partes é o que buscamos nesta etapa. Tornar equivalente os dois ramos mandibulares, checar movimentos de yaw, nivelamento, desvios, etc.

🇺🇸 Frontal view: Numerical Facial Analysis.

Here is something mathematical. After positioning the head to begin planning appropriately, we must pay attention to the deviations and asymmetries that may be present in the face.

Bringing the structures closer to the facial midline and trying to seek symmetry between the parts is what we seek at this stage. Make the two mandibular ramus equivalent, check yaw movements, leveling, deviations, etc.

🇧🇷 Uma vez escutei de um professor meu uma frase que me marcou muito quando eu me sentia ainda sem preparo técnico: “Operar? Isso a gente ensina até um macaquinho, é repetição!”

O que nos torna pessoas melhores é o quanto nos dedicamos a estudar, aprender, pensar…. Conhecimento sobre o assunto. E isso ninguém tira de você 🥰

O estudo e avaliação de cada face é o momento de maior importância dentro do planejamento de uma cirurgia ortognática. É onde precisamos do conhecimento aliado a um senso artístico de cada um para estipular parâmetros e decidir os movimentos cirúrgicos.

🇺🇸 I once heard a phrase from one of my professors that really resonated with me when I still felt unprepared technically: “Operate? We even teach this to a little monkey, it’s repetition!”

What makes us better people is how much we dedicate ourselves to studying, learning, thinking…. Knowledge on the subject. And no one takes that away from you 🥰

The study and evaluation of each face is the most important moment when planning orthognathic surgery. This is where we need knowledge combined with an artistic sense from each person to stipulate parameters and decide surgical movements.

🇧🇷 Vista de perfil desse caso espetacular 😍

Notem que a face é baseada em detalhes. Uma análise facial que é clássica e muito utilizada hoje é a do Arnett - Linha Vertical Verdadeira. Uso ela pra definição do “molde” do perfil independe da sua projeção anterior. Para mulheres o terço médio deve estar mais em voga do que o terço inferior da face. Nesse caso podemos ver que os lábios que antes estavam em distâncias iguais da TVL (true vertical line), após a cirurgia ficaram distribuídos com o lábio superior mais a frente que o lábio inferior… trazendo um ar mais feminino para o perfil e de boa projeção pro terço médio 😉

Note that the face is based on details. A facial analysis that is classic and widely used today is Arnett’s - True Vertical Line. I use it to define the “template” of the profile, regardless of its previous projection. For women, the middle third should be more fashionable than the lower third of the face. In this case we can see that the lips that were previously at equal distances from the TVL (true vertical line), after the surgery were distributed with the upper lip further forward than the lower lip... bringing a more feminine look to the profile and good projection for the middle third 😉

🇧🇷 Pra começar a semana esse caso mais do que impactante! Muito feliz em fazer parte desse processo 🩵

Lembro como se fosse ontem a primeira vez que ela chegou no consultório, já sabia de tudo sobre cirurgia ortognática antes de falar comigo a primeira vez. Embarcamos no processo, ela fez toda a preparação ortodôntica com e seguimos pra cirurgia. Hoje com praticamente 01 ano de pós, sem aparelho, sorrindo de um canto a outro… e quem agradece somos nós pela oportunidade de passarmos por esse processo juntos. Gratidão 🙏🏻
