How do we safely manage behavioral and psychological symptoms of dementia without overusing antipsychotics?

🌡️ Look for the trigger first. Check for pain, infection, constipation, dehydration, or medication side effects before labeling it “behavior.”

💊 Treat pain proactively. Unrecognized pain is a major cause of agitation.

🗣️ Use validation, not confrontation. Instead of correcting delusions, acknowledge emotions and provide reassurance.

🕰️ Maintain structured routines. Predictable daily schedules and consistent caregivers reduce anxiety and sundowning.

⚖️ Reserve antipsychotics for severe risk only. Use the lowest effective dose, shortest duration, with regular review and clear indication from your doctor.

Wernicke encephalopathy is a medical emergency caused by severe vitamin B1 (thiamine) deficiency that leads to acute brain dysfunction.

It classically presents with a triad of:
• Confusion or altered mental status
• Eye movement abnormalities
• Poor balance/ataxia

It most often occurs in people with alcohol use disorder, malnutrition, eating disorders, bariatric surgery, prolonged vomiting, or chronic illness.

Without urgent IV thiamine treatment, Wernicke encephalopathy can progress to Wernicke–Korsakoff syndrome, a permanent dementia with severe memory loss.
