We emphasize in our classes for this reason! “Informed consent is not easy. It is often not possible to convey all the knowledge required for informed consent in the limited amount of time available during a prenatal appointment or at bedside.

Pregnancy is an exaggerated vulnerable state and there is often limited capacity to absorb hard or scary information.

Professor Kenneth Boyd in the Journal of Medical Ethics says It is possible that the consent process acts as a “social filter,” placing vulnerable and excluded individuals at a disadvantage.

Boyd also points out that fully informed consent is not possible where the individual is vulnerable, either through illness, dependence or discrimination.

Knowing this, what can we as Doula’s and educators do? How do we hold sacred, the pillar of informed consent for families as they navigate the healthcare system? Slow down, slow everything way down.

“Is this an emergency or do we have some time to process what you’re saying?”

“Would you like some privacy?” “Would you like us all to leave the room while you think this over?”

“Do you have any further questions about the procedure? Do you need some time to do some research on you own and formulate those questions?”

Remind people that they are the expert over their own bodies and they are the experts over their babies. Even if the person has a white coat on and knows more than they do it is absolutely OK to ask questions.

Given adequate space, time, and information birthing people have a higher chance of saying yes when they truly feel like they understand what was happening and saying no, I don’t want that or I need more time or I’m in shock, scared and it is hard for me to take this in. If this is not an emergency I need 10 minutes to think about this.
