This November, for Diabetes Awareness Month, the Ottawa Valley Family Health Team celebrates the many ways we help our community manage diabetes safely and effectively—whether through our Diabetes Management Program, our partnership with Rideau Community Health Services, or other personalized support.

Our program helps patients:

💬 Access personalized support from our healthcare team
📅 Stay on track with check-ins and education sessions

As part of raising awareness and promoting proactive care, Lanark, Leeds, and Grenville OHT is offering a free diabetic foot screening clinic in Carleton Place. Early detection is key—taking care of your feet is an important part of managing diabetes.

✨ Our Carleton Place location has Moved! Thank You for Your Patience ✨

This past week, our Carleton Place team completed the transition to a single, centralized clinic location, and we’re thrilled to continue serving our community from our new home! 🏥💛

During our first week in the new building, we were able to see 844 patients—a true testament to the dedication of our staff and the understanding of our patients.

We also want to acknowledge that some patients may have experienced longer wait times on the phone or delays in scheduling during the transition. We sincerely apologize for any inconvenience and deeply appreciate your patience and understanding as we settled in.
