A recent CIHI report shows that 1 in 17 acute care admissions in Canada results in at least one episode of hospital harm, a reflection of ongoing workload strain and hospital overcapacity, not a lack of clinician care or commitment.

As patient care grows more complex and teams are asked to do more with fewer resources, the margin for error continues to narrow.
